Thank you for choosing The Happy Tail Rescue as your safe surrender place for your beloved companion. We do ask a Owner Surrender Fee of $20 per surrender or litter. Once The Happy Tail Rescue accepts your surrendered animal, you understand that you are no longer in custody of that animal. The Happy Tail Rescue will maintain adaquate care of the surrendered animal and will begin the process of finding your surrender a new home/family. If you are wanting to reclaim your surrender after a certain period of time, an adoption fee will be paid before returning to your possession.
By typing your name into the signature blocks below, your answers to this form attests that the information you have given is accurate and true. You also agree that, upon being accepted into THE HAPPY TAIL RESCUE's program, you will voluntarily relinquish all claims and ownership of said dog to the organization known as THE HAPPY TAIL RESCUE and members thereof. You understand that you cannot reclaim the dog once the dog is in possession of THE HAPPY TAIL RESCUE and understand that the animal will be spayed or neutered, if needed, in accordance with Louisiana state law. You agree to give up the dog's AKC registration papers (if available) and any and all medical records available. Neither THE HAPPY TAIL RESCUE nor members thereof shall be held responsible for the actions of the adoptive or foster family and/or dog. You furthermore certify that this dog is not vicious and has never shown signs of aggression towards human beings, unless otherwise noted above, and certify that this dog has never bitten anyone, unless otherwise noted above.
