Prior to the employee's last working date, the employee understands that the employee is required to return all company property items listed above to the Supervisor/Company Health & Safety Manager in good working condition as per above agreement. THIS Acknowledgement of Return of Company Property form has been completed together by myself (the employee) and Supervisor/Company Health & Safety Manager (identified below). The Supervisor/Company Health & Safety Manager has collected and inspected all company property listed above with me, and have noted on this form if there are any damages/issues with any of the returned property. If any payroll deductions are required due to damaged/issues with the company property returned, I will be contacted by the company shortly with the total balance which will be automatically deducted by my payroll. In the event the damages total balance (such as to a vehicle) exceeds my last payroll amount, I will be required to submit the required payment to the company as per terms agreed upon in a further communication.
