New Client Registration Form
Thank you for considering our hospital as your pet’s provider of veterinary services. We are dedicated to maintaining the health of your pet and look forward to many future years together.
Please complete this form as fully as possible prior to your first appointment which will help expedite the registration process and give us valuable insight in providing optimal care for your pet(s). The required sections have a red * asterisk.
A deposit for the first exam fee ($85.00) is required to book your first appointment as this reduces no-shows and last-minute cancellations that impact our ability to care for current patients. This is a non-refundable deposit that may be transferred to a rescheduled appointment if reschedule is made 48 hours (business days) in advance. Future appointments do not currently require a deposit.
If you have not already contacted us for an appointment, a staff member will reach out to you to schedule as soon as they’ve received your new client registration and set you up in our system.
Please have previous records (including doctors notes) sent to our practice PRIOR to your appointment. They can be faxed (585-624-7723) or emailed to us at info@honeoyefallsvet.com.
