    New Client Registration Form

    Thank you for considering our hospital as your pet’s provider of veterinary services. We are dedicated to maintaining the health of your pet and look forward to many future years together.

    Please complete this form as fully as possible prior to your first appointment which will help expedite the registration process and give us valuable insight in providing optimal care for your pet(s). The required sections have a red * asterisk.

    A deposit for the first exam fee ($77.00) is required to book your first appointment as this reduces no-shows and last-minute cancellations that impact our ability to care for current patients. This is a non-refundable deposit that may be transferred to a rescheduled appointment if reschedule is made 48 hour in advance. Future appointments do not currently require a deposit.

    If you have not already contacted us for an appointment, a staff member will reach out to you to schedule as soon as they’ve received your new client registration and set you up in our system.

    Please have previous records (including doctors notes) sent to our practice PRIOR to your appointment. They can be faxed (585-624-7723) or emailed to us at info@honeoyefallsvet.com.

    By checking “yes” I hereby grant Honeoye Falls Veterinary Hospital permission to use any photographs taken of myself or my pet, in any and all business publications, including but not limited to website entries, social media, and marketing materials without payment or any other consideration. I understand and agree that these materials will become property of Honeoye Falls Veterinary Hospital and will not be returned and hereby authorize to edit, alter, copy, exhibit, publish or distribute all photos for purposes of publicizing programs or for any other lawful purpose. I hereby acknowledge receipt of adequate consideration and waive the right to charge for use of the pictures, to inspect, or approve the images prior to any form of usage. I understand that the images may be modified to be used as design elements. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby release rights to all claims, demands, and causes to action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf of my estate have or may have by reason of this authorization.

    I hereby authorize Honeoye Falls Veterinary Hospital (HFVH) to render any treatment which is deemed critical to my pet's health while in their custody. I understand that in the event of any unforeseen or emergent circumstances, HFVH will make every reasonable attempt to contact me or my agent before proceeding with treatment. I understand that I will be financially responsible for all emergency procedures including the Estimate of Treatment provided in person or via telephone.

    All fees MUST be paid in full at the time services are performed or upon discharge from HFVH. Accounts will be turned over for collection if unpaid for more than 90 days. All costs of collection will be added to the outstanding balance.

    I hereby acknowledge when I schedule an appointment and fail to show, call or notify HFVH of my inability to keep this appointment time, it impacts their ability to provide services to both my pet and other clients. I understand a $55.00 fee will apply to my account if multiple appointments are missed without proper notification.

    I hereby acknowledge Honeoye Falls Veterinary Hospial's Zero Tolerance Policy. Anyone giving verbal abuse to members of staff, either in person, over the telephone or online will be sent a letter from our Practice Manager advising that this behaviour will not be tolerated and dismissed as a client of Honeoye Falls Veterinary Center.
