    This application is designed to help Casa Sheila and yourself determine if a specific dog is the right one for you and your household. Our top priority is to ensure our dogs will be well cared for.

    All adopting parties are required to complete this form prior to adoption. Casa Sheila reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions.

    Casa Sheila's adoption fee for dogs is $175 USD, which includes: disease testing/treatment, 2 rounds of deworming, age appropriate vaccines, Nexgarde (tick/flea treatment), spay/neuter and one set of travel papers (if needed).

  • Do you have young children or grandchildren living in or visiting the home often?*
  • Do you have other pets in your home?*
  • Do you understand that a dog needs yearly (and possibly emergency) veterinarian care and are you willing and able to provide that?*
  • Are you aware that a new environment can be stressful for your dog, and they may exhibit uncharacteristic behavior?*
  • Do you understand that rescue dogs can take many weeks to adjust to a new environment and are you willing to be patient with your new dog?
  • Are you willing to work through your dogs new issues, if any arise?*
  • Do you understand that adopting a dog is a lifetime commitment?*
  • If you do not get chosen for the dog you are approved for, yet are pre-approved... you can be added to the list for first-choice in the future. Would you like to be added, just in case?*
