Agreement and Informed consent statement. I understand that (1) The Option Method is designed to help me meet my goals of becoming a happier person by identifying the beliefs behind my unhappiness (2) the goal of Option Method training is to help me learn The Option Method, a self-help tool, (3) the role of The Option Method Teacher/Master Teacher is to conduct Option Method Dialogues and Option Method Dialogue Reviews, (4) The Option Method is not a form of psychotherapy or psychoanalysis and The Option Method Teacher/Master Teacher is not a licensed mental health care practitioner, (5) The Option Method Master Teacher, as an ICF credentialed coach, may document that the session occurred for recertification purposes.
I agree to the following policies and procedures of Dialogues in Self Discovery LLC:
Payment Procedure:
All fees are payable in advance. For pro bono sessions, I agree to allow the transcribed session to used as a teaching tools in social media, books and webinars.
Feedback: If, at any time, you, the student, feel that your needs are not being met or you are not getting what you want out of Option Method Dialogues, please tell me, so we can discuss your needs.
Dialogue Time: Dialogues are scheduled at the mutual convenience of The Option Method Master Teacher and The Student. The day and time for the next Dialogue will be scheduled at the close of each Dialogue. All Dialogues will begin at the scheduled time and end one hour later.
Call Procedure: If The Student opts to do Dialogues by phone or internet, The Student will contact The Option Method Master Teacher at the pre-arranged time as scheduled. Calls/Zoom sessions may be recorded upon the agreement of both parties.
Cancellations: Please give forty-eight (48) hours prior notice if you need to cancel or change the time of an appointment, otherwise you will be charged for the Dialogue in full. The Option Method Master Teacher will make reasonable efforts to reschedule Dialogues which are cancelled in a timely manner.
Confidentiality: Student information and dialogue content will be held confidential, unless otherwise agreed to by the student and teacher (and guardians/parents, if student is a minor), or as required by law and ethical practices.
