Downtown Denver Expeditionary has taken reasonable steps to provide your child a safe, educational and enjoyable experience. We wish to remind you, however, that no activity is without risk. We do not want to frighten you or reduce your enthusiasm for these activities, but it is important for you to be informed of the possible risks. The following activities listed describe some, but not all, of those hazards and risks:

 

Urban Experience – Hazards can include the risks associated with contact with strangers, separation from the group, traffic, crime, physical hazards, crowds, severe weather, fire, transportation delays, transportation accidents, animals, water hazards, refuse or trash, purchased food.

 

Outdoor Experience – Hazards can include the risks associated with transportation to and from the activity, separation from the group, altitude sickness, exposure to the elements, proximity to help, cold/heat exposure, animals, severe weather, allergies, exposure to strangers, terrain, fire, avalanche, falling objects, outdoor hygiene.

 

Camping Experience – Hazards can include those listed under Outdoor Experience plus the risks associated with packaging and cooking foods, working with camp stoves, and overnight camping.

Acknowledgment of Risk, Release & Hold Harmless Agreement:

I agree to allow my child to participate in the activities outlined above. I am aware of the hazards and risks that are possible in these kinds of activities and agree that, to the full extent permitted by law, for myself and my child, I RELEASE and hold Downtown Denver Expeditionary School, its employees and agents, and the parent, teacher, and staff organizers (together “the School”) harmless from any liability resulting from injury sustained to my child while participating in these activities.

I also agree to defend and INDEMNIFY against any claims, damage, loss or expense of whatever kind or nature that the school may have to pay that arises from by child’s intentional, grossly negligent, or reckless acts or omissions while participating in this activity. 

I further authorize the School, through any agent or employee who is supervising my child while participating in such activities to require my child to comply with any rules, standards of behavior or instructions such employee(s) or agent(s) may reasonably establish including those outlined in School student or parent handbooks and Board Policies.  In the event that my child has violated the rules and is being sent home by the School’s agent, I understand that the cost of transporting my child back home may be my responsibility.

Authorization & Consent to Seek Urgent Medical Care

I hereby authorize the employee(s) or agent(s) of the School who is supervising my child to act on our behalf in authorizing and consenting to emergency medical care, dental care, and/or hospitalization for said minor if he/she becomes ill or is injured while participating in the activities listed above. This Authorization and Consent may be presented to the appropriate medical/dental staff at such time as emergency medical care, dental care or hospitalization is required.

I, the undersigned parent(s)/guardian of the minor student, hereby authorize the employee of the school supervising the activity, as my agent, to consent for my child to any x-ray examination, anesthetic, dental, medical or surgical diagnosis or treatment, and hospital care which is deemed advisable by, and is to be rendered under general or special supervision or, any physician and surgeon on the medical staff of any licensed hospital whether such diagnosis or treatment is rendered at the office of said physician or at the hospital.

It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required but is given to provide authority and power to our agent to give specific consent to any and all such diagnosis, treatment or hospital care which a physician in the exercise of his/her best judgment may deem advisable.

Also, the authorized the school’s employee has the authority to call for emergency medical transportation or provide transportation him/herself, for the benefit of the involved student, as the employee deems necessary.  The School and its agents will make every reasonable effort to contact parents in connection with any necessary medical care or transportation for their child.
