DLB Goalie Academy Scholarship Application

Application Deadline - June 24, 2022
Read and follow the instructions carefully prior to submitting your application. All finalists will be notified on or before the 15th of July 2022.
If selected, you may be asked for further documentation to support the data you provide. All correspondence and award agreements will be generated from the personal information submitted below.

Partial Awards will be given to the student-athlete upon successfull completion of the first semester and the remainder after successfully enrolling in the 2nd semester. Student must be in good standing with the school and further documentation may be required. If the recipient does not submit the appropriate documentation to fulfill the requirements to receive the award by June 1, 2023, all undelivered funds will be forfeited.
