We are so pleased and grateful that you have chosen our rescue to donate your time to! By signing this document you agree that you have read and understand all of the conditions and statements below.This form is to be completed by the parent or guardian of any minor child who is applying to volunteer.Read each of the following statements and click the box next to them.

AT WILL. Your volunteer position is temporary and is not guaranteed for any specified period or term and constitutes an at will arrangement. As a result, you are free to resign at any time, for any reason. Similarly, the Rescue is free to conclude its relationship with you at any time, with or without cause, and with or without notice.
PROFESSIONALISM. It is imperative that all representatives of the Rescue always conduct themselves in a professional and respectful manner. Representatives of the Rescue, including all volunteers, agree not to conduct themselves in a way that reflects poorly on the Rescue. If you have questions about what conduct is considered inappropriate, please contact a representative of the Rescue.
COMPENSATION. The Rescue will not provide compensation for volunteer positions.
LIABILITY. The Rescue shall not be liable for any damages, injury, or losses that may occur during your tenure as a volunteer. You assume the risk of any such damages or losses.
CONFIDENTIALITY. During the course of your volunteerteerism, you may have access to certain proprietary or confidential information, technology, data, methods, files, records, supporter lists, and other documents such as memos, correspondence, donor information, administrative tools, priveledged documents, etc. You may also have access to information concerning the identity of individuals and facts regarding the Rescue's supporters. This information is strictly confidential and must not, under any circumstances, be disclosed to anyone. You agree to keep any such information confidential. Volunteers who disclose any information will be subject to termination. You agree not to disclose or discuss any information learned during your volunteerism with anyone. This provision will stay in effect even after termination of the volunteer contract.
DISPARAGEMENT. I agree that, as part of the consideration for volunteering, I will not disparage the Rescue or its foster or adoption programs, practices, policies, or procedures.
LEGAL FEES. I agree and understand that an breach of the confidentiality provision will "per se" damage the Rescue, in the event any legal action needs to be taken regarding a breach of this agreement or the confidentiality provision, I understand that I may be liable to pay the organization liquidated damages in the amount of $1,000.00. I will also be liable to pay the organization's costs and reasonable attorney's fees in enforcing this agreement.
