If selected into the program I agree to the following Volunteer Waiver & Agreement.
If Applicant is 16 to 17 years old:

I, as the parent/guardian, give consent to The Backstage Foundation to run a background check on my child, age 16 or 17 yrs. oldI, as the parent/guardian, do not give consent to The Backstage Foundation to run a background check on my child, age 16 or 17 yrs. old.
If Applicant is 18 yrs. or older:

I, as a non-minor volunteer applicant, acknowledge that The Backstage Foundation requires a background check on volunteers 16 yrs. & older.
Signatures

PLEASE NOTE: A signature is required from volunteer applicants age 16 and older. In addition, a signature is required by a parent or guardian for applicants under the age of 18 years old.
By signing & submitting your Volunteer Application / Waiver & Agreement, you are certifying that all information is correct and that you are the person completing this application for you or as the parent/guardian. In addition, by signing this application / waiver & agreement you are certifying that you have read and fully accept the terms and conditions of this application / waiver & agreement. When you press the submit button, you will receive an email copy of your volunteer application / waiver & agreement.
