Rating Instructions:
Rate observed behavior by numerical value definitions contained in the Standard Evaluation Guidelines of the Field Training Manual. You must comment on the Most and Least acceptable performance of the day.
You are encouraged to comment on any behavior you wish, but a specific comment is required on all ratings of "1" or "7."
NO-not observed. All skills start here for the shift.
1-Totally unacceptable, requires documentation
2-Close to a one, just better than totally unacceptable
3-Moving closer to a four, but not yet minimally acceptable
4-Represents minimally acceptable performance. A “4” is a fully trained solo officer just out of the program, operating acceptably without guidance or direction. Is not based upon a tenured officer.
5-Performance above standard, like a bonus score
6-Closer to 7, still a bonus score
7-Represents superior performance, requires documentation
RT-Remedial Training
• Repeated failures
• Substandard work, 3’s and below
• Safety infractions/Policy violations
• Difficulty understanding concepts
• Makes excuses
• Denies problem is present
• Doesn’t take responsibilityNRT-Not Responding to Training. Probationary Officer has not improved with remedial training in a specific topic. This a repetitive action or non-action that shows the officer is deficient in this skill. Requires documentation and FTO Coordinator must be notified.
