It is the policy of the Whole Health Dental Center to document and charge a fee for
missed appointments without 24 hour notification. The patient fee is determined by
whether the appointment missed was the first “no-show”. Please reschedule your appointment as soon as possible to ensure proper dental care can communication with your provider. A cancellation code will be given at the time of cancellation provided the 24 hour guideline is met. Please keep this cancellation code for future reference. To reschedule any appointment, please call our office.

Any patient who fails to arrive for a scheduled appointment with either the dentist or
hygienist without notice 24 hours prior to the scheduled appointment time is considered a “no-show”. There is no charge for the first “no-show”, after which the patient is charged $50.00 for each additional “no-show” per year.
