River Valley Veterinary Services shall exercise reasonable care for pets delivered for boarding.

In the event of injury or illness, River Valley will attempt to contact the animal’s owner for instruction. If the owner cannot be reached, River Valley will attempt to contact the emergency contact. If neither the owner nor emergency contact can be reached, River Valley will move forward with any medical treatment recommended by River Valley veterinarians. By signing below, the owner authorizes any such medical treatment and acknowledges their agreement to pay for any needed medical services.

In signing this agreement, the pet owner agrees that he/she is aware of risks from contact with other animals, including but not limited to scratches, bites, and communicable diseases.

The owner agrees to be solely responsible for any and all acts and behavior of their pet, included but not limited to biting other animals and humans. The pet owner has disclosed any history of aggression towards other animals and humans.

The owner agrees to permit River Valley Veterinary Service to use any pet images and or video for promotional purposes.
