Lady Juno's Conditions of Play

 Playthings must fill out this sheet to the best of their ability before a session can commence. Information is power and the more detailed you are in your responses the more personalised the session and the better the experience. 

 All Information will be kept archived securely. Adjustments will be made to your play preferences as sessions progress to reflect your experience level and evolving tastes.

 Our sessions are between us and confidentiality is key as Mistress aims to ensure a safe environment in which we can explore. 

 Future sessions may be open to discussion, customisable to different niche tastes and experiences depending on loyalty and trust. Loyalty and Service are highly regarded and often rewarded.

By filling out this document you are entering into a profesional relationship with Lady Juno and must adhere to her rules and regulations. If this is not filled out correctly or boundaries are pushed you will be forgotten. 

Medical History

It is vital that you disclose any and all medical Conditions both Mental and Physical to ensure Safe, Sane and Consensual play for both parties. 

Disclosing this information does not mean missing out, rather it allows Mistress to make concessions and adapt the experience to better fit these requirements . 

This may include but is not limited to,

Heart conditions, chest pain, fainting spells, dizziness, or high blood pressure. All surgical implants, including pacemakers. Diabetes, asthma, respiratory and breathing difficulties must be disclosed. High or low blood sugar problems. Claustrophobia, muscular and skeletal issues, injuries, broken bones, particularly neck and knee issues. Allergies to latex, rubber, talc, and nonoxynol-9. Contact lens wearing, cataracts, and glaucoma.

Any and all mental health issues that you have suffered with, been diagnosed with, taken medication for, including addictions. It is your responsibility to disclose any of the above or anything else that may not be listed.

Mistress is up to date and qualified in First Aid and CPR. The best safety is preventative however so assess yourself and your limits carefully and communicate clearly. 

INFORMED CONSENT STATEMENT;

I am requesting and at my own risk participating in a BDSM session. I understand activities within the Session will be conducted by mutual consent. I accept sole responsibility to; (a) decide what activities I wish to participate in during the Session and decide what my boundaries are. (b) communicate these decisions clearly before the start of the Session. (c) agree to an appropriate safe word or other signal to request an activity stop. (d) signal immediately, any unexpected fatigue, shortness of breath, chest discomfort, nausea, pain or discomfort. I understand I can discontinue participation at any time during the Session, without prejudice against me, beyond fees or tribute for the Session. LIMITS OF LIABILITY; I understand that any activity undertaken during the Session are voluntary and are at my own risk. I agree to indemnify and hold harmless all participants in the Session from all claims of liability, injury, death, property loss or damage, attorney’s fees and costs and I agree to reimburse them for any such claims; of same, they may have. AGREEMENT; by checking the box and submitting this session application, I acknowledge I have read, understood and agree to the contents of this form and I confirm that all information given by me, is correct to the best of my knowledge. I understand there is no sex involved in Mistress Sterling's sessions, I am a consenting adult over the age of 18 and I am not a law enforcement officer or federal law enforcement agent.

Our sessions together should be enjoyable, even if that means suffering for pleasure. If at any time you want to back out of something in a session you will be respected for it no matter the reason or act. Boundaries are important on both sides and should be respected.

Green - "Even if I sound like I hate it I love it"

Orange - "Getting very uncomfortable, approaching my Limit"

Red - "Stop what you're doing"

Purple - "Stop the entire session" 

If you have filled this out to the best of your ability Mistress will process it along with a picture of photo Id held up next to your face for verification. Do not obscure any information. This is vital for the vetting process. 

You will then recieve a confirmation email along with a link to book a session.
