I consent to having an eyelash lift (perm) and/or eyelash tint and re-touching of the eyelash lift and/or eyelash tint applied to natural eyelash performed by the assigned artist. 

 

I understand that there are risks associated with eyelash lift (perm) and/or eyelash tint procedure. I further understand that as part of the procedure, eye irritation, eye pain, eye itching, discomfort, and in rare cases eye infections or blurriness could occur.

 

I understand and agree that if any of these issues persist, I will contact the artist and/or consult with a physician at my own expense. 

 

I agree to the aftercare instructions given to me and realize that if the failure to follow these instructions may result in the decreased life of the eyelash lift and/or eyelash tint.

 

I was given the oppurtunity to ask the artist any questions I have regarding the service and I have had those questions answered to my satisfation. 

 

This agreement will remain in effect for the procedure and all further procedures conducted by my artist at Luminary Aesthetics & Co. I understand that this agreement is binding and that I have read and fully understand all information above.
