As payment for the OTbeat Burn, I hereby authorize  Location   d/b/a Orangetheory® Fitness, an independently owned and operated Orangetheory® Fitness franchised location (“OT), to charge the card specified below in the amount of $39.67 plus applicable tax on:
TODAY (Date) and (   Date) and (   Date    ). If the noted payment date(s) fall on a weekend or holiday, I understand that the payment may be charged on the next business day.
I agree to notify OT promptly of any changes in my method of payment. I understand that if I fail to satisfy a payment specified above, OT may require immediate payment of the total outstanding balance for the OTbeat Core and may engage in collection efforts to collect such payment.
I understand that there are no financing charges associated with this purchase. I also understand that neither OTF Franchisor, LLC or OTF Royalties, LLC the entity or entities who granted OT contractual authority to independently own and operate this franchised location, nor any of their past, present, or future affiliates or subsidiaries, or their respective officers, directors, incorporators, members, partners, owners, agents, management, controlling parties, entities under common control, vendors, service providers, investors, attorneys, employees, or representatives (collectively referred to as the “Franchisor”) is a party to this agreement. I understand and agree that the Franchisor is not responsible for any acts or omissions related in any way to this agreement.
