Parental Consent: I hereby consent to let my child(ren) listed above participate in the "Come to the Table" VBS. It is understood that every precaution will be taken for the safety and well-being of my child. Still, in the event of accident or sickness, The Diocese of Toledo, Saint Mary, and Saint Michael Parishes, its staff, and its volunteers are at this moment released from any liability.

Cost is $10 per child which includes a T-shirt. 

Payment can be made by putting your fee in an envelope marked VBS with your family name on it and place in the collection basket at Mass or, stop in and pay at either parish office.

By submitting this form, I give consent to Saint Mary and Saint Michael to publish photos of the above named child in groups of two or more on the parish's social media sites and in print form.
