Liability Release Form I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of the activities offered. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Salted Performing Arts and Salted Dance Company LLC, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Salted Performing Arts and Salted Dance Company LLC”. I hereby agree to release Salted Performing Arts and Salted Dance Company LLC harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant. I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the risk of injury and death. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation. I also give Salted Performing Arts and Salted Dance Company LLC permission to use my child’s picture in or on any form of advertisement for Salted Performing Arts and Salted Dance Company LLC affiliated events. If I am a minor, my parent and/or legal guardian has signed this document releasing Salted Performing Arts and Salted Dance Company LLC from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The participant has my permission to participate in Salted Performing Arts and Salted Dance Company LLC. I warrant the below information is complete and correct. I further release Salted Performing Arts and Salted Dance Company LLC of all liabilities associated with my child’s participation with Salted Performing Dance Online.
Safety, Arrival, and Dismissal
To ensure the safety of students and staff, parents are not allowed into the building(there is no waiting room). During classes, the building doors stay locked during class.
-Drop off and pick up is at the door. The student’s pin must be used to drop off and pick up a student. If said pick-up person does not have the student's pin, pick-up nor drop-off is allowed.
- To ensure safety building doors will be locked 15 minutes after class starts and students will not be able to enter unless the reason for the delay was given before class.
Attendance and Class
There are no refunds, adjustments, or credits for missed classes, even if the student is ill.
If a student missed an excessive amount of classes this could affect their eligibility to perform in SPA concert.

No food, drinks, or snacks will be allowed in the classroom other than water.
Please note that classed schedules are subject to due to enrollment and staff availability.
Salted Performing Arts requires that all dancers wear the appropriate attire for specific classes. Please mark your dancer’s name on all of their belongings to prevent mix-ups during in-class. No jewelry, with the exception of stud earrings, should be worn in class. For Safety nails are not to be longer than the nail bed.
Salted Performing Arts is not liable for any missing or stolen items.
Late Pick Up
After 5 minute grace period. Members who are late picking up their students will be charged $15 every 15 minutes that they are late.
Cellphones and Socia Media
Students are not allowed to have cell phones out while in the building. Cell Phones must be turned off and must stay in the student’s bag. If a student does not have a bag, the cellphone turned off and be put in the cellphone bucket until dismal. While Salted Performing Arts encourages and makes space for friendships to be made between students; Salted Performing Arts is not responsible nor liable for events between students and anyone else on any social media platform. Students and their families should conduct themselves in a professional and kind manner.
Charge Back Policy:
Salted Performing Arts treats its customer with the utmost respect and care. However, if a financial dispute arises and a client files a chargeback fee. Salted Performing Arts have the right to challenge the chargeback fee filed against them. Client’s who file chargeback disputes will be immediately released from Salted Performing Arts and barred from returning to the organization.
Photo Release:
I acknowledge, I irrevocably grant to Salted Performing Arts and Salted Dance Company LLC and Company’s
assigns, licensees, and successors the right to use my child’s/ ward’s image and name in all forms and media including composite or modified representations for all purposes, including advertising, trade, or any commercial purpose throughout the world and in perpetuity. I waive the right to inspect or approve versions of my image used for publication or the written copy that may be used in connection with the images.I release Salted Performing Arts and Salted Dance Company LLC, Company’s assigns, licensees, and successors from any claims that may arise regarding the use of my image, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright. The company is permitted, although not obligated, to include my name as a credit in connection with the image. Salted Performing Arts and Salted Dance Company LLC is not obligated to utilize any of the rights granted in this Agreement.
Illness/Injured Child Policy
For the protection of all students, staff, and their families, please do not bring your student to dance if you observe any of the following symptoms:
Nasal discharge that is green or yellow
Complaints of ear pain
Consistent cough
Severe sore throat
Eyes that are pink, burning, itching, or producing discharge
Diarrhea or vomiting
Fever
Dancers must be symptom-free for at least 24hrs following fever, vomiting, or diarrhea before returning to class. Call-ins are appreciated!
If a dancer is injured in class, a report will be filled out at the front desk and a parent will be notified. Injuries that occur outside of dance should also be reported to the front desk. A doctor’s note may be required before the dancer returns to class, depending on the injury. Students with injuries are permitted to sit and observe class until they are cleared to participate.
Salted Performing Arts cannot dispense medications of any kind.
Autopayment
All students must be enrolled in autopay to attend Salted Performing Arts. Accounts will be processed on the 15th of every month to pay for tuition and fees. To opt out of auto-pay Salted Performing Arts must be emailed two weeks before the next payment period

I understand the first process amount will process the enrollment fee and T-shirt.
I understand to have a membership with and participate in activities and events of Salted Dance Company LLC and Salted Performing Arts, I must be enrolled in reoccurring monthly auto-payments.
I understand I must keep an active credit card on file at all times.
I understand if my payment is late I will get charged a $20 late fee and my card will get processed again.
I understand missing membership payments may result in my student losing their spot in class and may result in membership cancelation.
I understand if I want to opt out of my membership I must email Salted Performing Arts 30 days before requesting membership cancellation and wait for a confirmation email.
I understand membership fees will be processed on the 15th of every month.
I understand that if I enroll in the middle of the season I will pay for the enrolment fee, and t-shirt when joining.
I understand this is a membership and continues to renew every month unless canceled.
Salted Performing Arts reserves the right to refuse service, expel, and/or admission to any individual. All schedules are subject to change based on enrollment and availability of staff. A Salted Performing Arts representative will contact those involved in the cancellation, rescheduling, or combining of any classes.

Salted Performing Arts conducts itself in a way that is pleasing to Our Lord and Savior Jesus Christ. ( Christian Biblical Standards, Holy Bible ESV) With that being said we expect our members to hold, respect, and honor these values.
