  • This form serves as the Final Report for the 2022-2023 American Rescue Plan Act (ARPA) Food Support Grant for food shelves, on-site meal programs, and Tribal nations. Budget documentation in the form of receipts and source documentation must be dated within the grant period 5/1/2022 to 6/30/2023 and must be available upon request. Budget documentation must equal grant amount and indicate what the funds were used for. All grant related documentation must be retained for 7 years.

    Funds must be spent by June 30, 2023.

    This final report must be completed by July 15, 2023.

    Each grant recipient is required to complete this final report. For example, if an organization has multiple brick and mortar sites and received separate ARPA funds for each site, you are required to submit a report for each grant received.

    By signing this final report form, the above named grantee attests to their use of  ARPA Food Support grant funding in accordance with Federal, State & Local laws, rules & regulations and terms and conditions as required by Hunger Solutions Minnesota and the State of Minnesota as detailed below:

    1.  Grant funding was used to support allowable food program related expenses, focused on COVID impact.

    2.  Grant funds were subdistributed to any agency beyond the applicant.

    3.  ARPA Food Support Grant funding was used by June 30, 2023.

    4.  Grantees abided by all CDC and MDH health and safety recommendations in regards to COVID-19. This applies to all food distributions, staff and volunteer work, and participant and community interactions.

    5. Receipts must be available upon request that equal grant amount and indicate how funds were used.

    6. Food was distributed to persons in need at no charge regardless of race, color, religion, creed, national origin, sex, marital status, status with regards to public assistance, membership or activity in a local commission, disability, sexual orientation, age or other criteria unrelated to need.

    7. The food program did not require ID, proof of residency/address, or proof of income from participants.

    8. The food program was not used to foster or advance religious or political views.

    9.  The food program had a stable address, updated phone and website information, and provided direct service to individuals and families.

    10. The food program was accessible to people with a disability and has a plan in place to serve disabled participants in compliance with the Americans with Disabilities Act (ADA).

    11. The food program made voter registration materials available to participants. https://www.revisor.mn.gov/statutes/cite/201.162  (Not applicable Tribal Nations).

    12.  The food program made SNAP food support materials available to participants. www.hungersolutions.org/store  (Not applicable for Tribal Nations).

    13. The food program responsibly and properly handled and safeguarded participant information and received required permission before sharing any personal information including names and addresses.

    14. The food program repaid any funds if there was discrimination in service or if there was failure to comply with other terms and conditions of the contract.

    15. All grant related documents will be retained for 7 years.
