Kids Lion Dance Culture Summer Camp Registration

Summer camp cost: $250 (included registration fee, lunch and snack, summer camp T-shirt and craft material) 

Ways of payment:

1.  In person: fill the registration form below, go to the front desk of Guangming Temple, pay in cash or check. Please write student's name and camp name on the memo. The registration process is complete.

2. Sent check by mail: Fill the registration form then mail the check to Guang Ming Temple (Address above). Please make the check payable to I.B.P.S. Please write student's name and summer camp name on the check memo. The registration process is complete.

3. Use paypal:

Login in your paypal account, use Orlando@ibps.org to find Guang Ming Temple, and pay the camp fee. Please use the memo to write students name and camp name and then forward the paypal receipt to Orlando.Chineseschool@ibps.org with student's name and camp name. A follow-up email will help us to find your payment. 

Another way use paypal is to go to Temple donation webpage link (below) send the camp fee https://www.orlandobuddhism.org/online-donation then forward the receipt to Orlando.Chinseseschool@ibps.org so we know you finish the registration process. 

Please pay the camp fee within one week of filling out the registration form so we can hold your spot.

EMERGENCY AUTHORIZATION & WAIVER OF CLAIMS 授權急救及免責聲明:  

I request that the above named student be permitted to participate in the Guang Ming Chinese School (GMCS) activities. He/She is in  good physical condition, but in case of illness or accident, GMCS has my consent to secure necessary medical attention for the above named  student. I, (please print your full name)___________________________________, hereby release and discharge GMTCS, its directors, officers,  teachers, instructors, staff members, and all other associated entities (the “Release”) from all claims, demands, costs, expenses, actions and  causes of action, whether in law or equity, in respect to death, injury, loss or damage to the above student or his/her property however caused,  arising out of the above student’s participation in activities at or related to the School, whether or not on the School’s premises. I hereby  indemnify and hold each of the Release harmless from any and all damages, costs, losses and expenses suffered by him/her arising out of such  participation and activities.  

CONSENT: I give my consent to Guang Ming Chinese School to use any of the photos taken during my child/children’s participation in any  of the activities of GMCS for the purpose of communicating, promoting, fundraising and, or other information about the class and the school as  a whole.
