    2023 Stake Young Women Camp Registration

  • IMPORTANT NOTE: This form is to be filled out jointly by the participating young woman and their parent or guardian.

     

    Dear Young Women and Parents,

    We look forward to being together this summer at Zions Camp!

    Please use this form to register each youth for Young Women Camp. If there is more than one young woman in your family, complete the form once for each.

    There are three steps to the registration process:

    1. Provide parent/guardian information
    2. Provide youth information
    3. Complete the acknowledgements and agreements

    As you complete each step you will be automatically taken to the next step.

    REGISTRATION FOR YOUNG WOMEN CAMP WILL CLOSE ON JUNE 1.

  • I give permission for my daughter to participate in the Nashua Stake Young Women Camp and authorize the adult leaders supervising this event to administer emergency treatment to the above named participant for any accident or illness and to act in my stead in approving necessary medical care. This authorization shall cover this event and travel to and from this event.

    I grant the Nashua New Hampshire Stake the right and permission to use and publish images, photographs, video, and audio recordings made of me or my child at all stake youth activities, and I hereby release the stake, the activity coordinators, and all related parties from any and all liability from such use and publication. I further authorize the reproduction, exhibit, broadcast, electronic storage, Internet streaming, and/or distribution of such images, photographs, video, and audio recordings without limitation at the discretion of the stake.

  • As a youth participating in the 2023 Nashua Stake Young Women Camp, I am responsible for my own conduct and am aware of and agree to abide by Church standards, event safety rules, and instructions from Church and Camp leaders. My parents and I understand that participation in this activity is not a right but a privilege that can be revoked if I behave inappropriately or if I pose a risk to myself or others.

    I will strive to follow the Savior Jesus Christ by treating others with love, respect, and kindness. My conduct and interactions will abide by Church standards and exemplify Christlike behavior. I will live by the principles in "For the Strength of Youth."

  • Outerwear and Safety Guidelines

    "The Lord’s standard is for you to honor the sacredness of your body, even when that means being different from the world. Let this truth and the Spirit be your guide as you make decisions. . . . Be wise and faithful, and seek counsel from your parents and leaders." (FSOY, p. 27)

    Youth and Adult campers should bring/wear:

    • Closed-toe shoes (shoes must be worn throughout camp & to and from the beach)
    • Swimsuits must cover the stomach, one and two-piece suits that maintain this standard are permitted.  Changing stations will be available at the beach.  If youth or leaders need to walk up to camp in their swimsuit, they should have a towel wrapped around them.

    Our desire is that each youth who attends camp will feel their Savior’s love and increase their testimony of Him.  We want each young woman to leave camp knowing that she is a beloved daughter of Heavenly Parents and having confidence to ask for and receive revelation from her Heavenly Father.

    FAQs . . .

    Can we wear open-toed sandals and flip flops?  

    • Nope.
      • All YW and leaders are required to wear shoes to and from the waterfront.  For safety reasons, closed toed shoes or sandals of a sturdy nature are recommended.
      • We recommend Crocs or water friendly shoes set specifically for this purpose. For those visiting camp for the first time, there is a steep, rocky, dirt path leading to and from the waterfront.

    What are the guidelines for the waterfront?

    • Swimsuits must cover the stomach. One and two-piece suits that maintain this standard are permitted.
    • There are changing areas at the beach.  If a YW or leader needs to walk up to camp in her swimsuit, she should have her towel wrapped around her.
    • All Young Women and leaders are required to wear shoes to and from the waterfront (see above).

    Are tank tops allowed?

    • No tank tops, please.
      • Beyond modesty, protection from the sun, bugs, and elements is key when outside.

    Other Suggestions:

    • Protect yourself from the sun.
      • Apply sunscreen regularly, wear a brimmed hat, wear light, loose fitting clothing to protect you from the sun and keep you cool
    • Protect yourself from ticks and other bugs.
      • Apply bug repellent regularly, wear longer sleeves and pants while in the woods.

  • Youth Boating Activities Release, Waiver, Indemnification, Hold Harmless, and Assumption of Risk Agreement

  • In consideration of being permitted to participate in tubing, water skiing, wake boarding, knee boarding, or any other related activities that involve riding in or being towed behind a ski boat (“Boating Activities”) owned and operated by Daron L. Janis of Peterborough, NH (“Boat Owner”) as part of the Young Women’s Camp of the Nashua, New Hampshire Stake (“Camp”), the undersigned parent or guardian (“Undersigned”), acting on behalf of the youth participant named below (“Youth”), agrees to the following:

    1.  The Undersigned and the Youth fully understand that (a) Boating Activities involve risk and danger of serious and non-serious bodily injury, including, but not limited to, bumps, cuts, bruises, broken bones, whiplash, permanent disability, paralysis, and death (“Foreseeable Risks”); (b) these risks and dangers may be caused by the Youth’s own actions or inactions, the actions or inactions of other participating in the Boating Activities, the conditions in which the Boating Activities take place, or the negligence of the Boat Owner or Camp staff or volunteers; and (c) there may be other risks and social and economic losses either not known or not readily foreseeable at this time to me or the Youth (“Non-Foreseeable Risks”).  The Undersigned agrees, represents, and warrants that the Youth is qualified, in good health, and in proper physical condition to participate in Boating Activities at Camp;

    2.  The Undersigned acknowledges that the Youth will be under adult supervision by Camp staff, that the Boat Owner is not responsible for the conduct and behavior of the Youth, and that any first aid or medical care is the responsibility of Camp staff.  The Undersigned grants the Boat Owner and Camp staff or volunteers permission to take whatever actions they deem necessary to assist the Youth during Boating Activities at Camp in the case of an emergency or unsafe situation, and to act immediately to preserve the safety of the Youth.  This may include terminating Boating Activities or momentary touching of the Youth;

    3.  The Undersigned fully accepts and assumes all Foreseeable Risks and Non-Foreseeable Risks associated with the Youth’s participation in Boating Activities at Camp, and fully accepts and assumes all responsibility for losses, costs, damages, and attorney’s fees that may be incurred as a result of the Youth’s participation in Boating Activities at Camp.  The Undersigned and his/her executors, administrators, assignees, or heirs (collectively, “Releasors”), further agree to release, waive all liability against, indemnify, hold harmless, and defend the Boat Owner and Camp employees, directors, officers, agents, and volunteers (collectively, “Releasees”), from any and all fault, liabilities, costs, expenses, losses, damages, claims, demands, or lawsuits of any kind arising out of, related to, or connected with the Youth’s participation in Boating Activities at Camp.  The Releasors further agree that if, despite this release, waiver of liability, assumption of risk, and indemnity agreement, any claim, demand, or lawsuit arises or is asserted by a Releasor, or by anyone on a Releasor’s or the Youth’s behalf, that in any way whatsoever is related to the Youth’s participation in Boating Activities at Camp, whether arising under the laws of the United States or of any State, or under the theory of law or equity, the Releasors will indemnify, hold harmless, and defend the Releasees from any and all costs, expenses, or liability including, but not limited to, the cost of any settlement or judgment made or rendered against a Releasee, whether individually, jointly, or severally, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand, or lawsuit, including attorney’s fees.

    THE UNDERSIGNED HAS READ THIS AGREEMENT, FULLY UNDERSTANDS ITS TERMS, UNDERSTANDS THAT HE/SHE HAS GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, HAS SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE, AND INTENDS IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

  • Youth Firearms Instruction Release, Waiver, Indemnification, Hold Harmless, and Assumption of Risk Agreement

  • Whereas, in return for instruction in firearms, use of premises, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the undersigned parent or guardian (hereinafter “The Undersigned”), acting on behalf of the named youth participant below (hereinafter “Youth”) at 30 Onway Lake Road, Raymond, NH (hereinafter “Camp”), agrees to the following:

    The Undersigned agrees to indemnify, hold harmless and defend certified Range Safety Officer Derek J. Fry of Brookline, NH, (hereinafter “The Instructor”) and any of his employees, directors, officers, agents, assistants, or vendors and owners/operators/volunteers of Camp facilities used by the Instructor, from any and all fault, liabilities, costs, expenses, claims, demands or lawsuits arising out of, related to or connected with: the discharge of firearms; the course of instruction; the Youth’s participation in the course of instruction; the range, buildings, land and other associated facilities of the Camp used for the course of instruction; the Youth’s presence on or use of said Camp; and any and all acts or omissions of The Undersigned or Youth. And should any such claim, demand, or lawsuit arise or be asserted in any way whatsoever related thereto, whether arising under the laws of the United States or of any State, or under the theory of law or equity, The Undersigned will indemnify, hold harmless and defend The Instructor from any and all costs, expenses or liability including, but not limited to, the cost of any settlement or judgment made or rendered against The Instructor, whether individually, jointly, or in so lido with the Undersigned, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand, or lawsuit, including attorney’s fees. 

    The Undersigned furthermore waives for himself/herself, any and all rights and claims for damages, losses, demands and any other actions whatsoever, which he/she may have or may arise against The Instructor, (including, but not limited to any and all injuries, damages or illnesses suffered by the Youth, or the Youth’s property), which may, in any way whatsoever, arise out of, be related to or connected with: the course of instruction, the Camp, including any latent defects in said Camp; the Youth’s presence on or use of said Camp; the Youth’s property (whether or not entrusted to The Instructor); and, the discharge of firearms. The Instructor shall not be liable for, and The Undersigned, on behalf of himself/herself, hereby expressly releases The Instructor from any and all such claims. The Undersigned hereby expressly assumes the risk of Youth’s entering the Camp and of taking part in shooting activities at said Camp, which include, but are not limited to, instruction in the use of large and small caliber firearms, the discharge of such firearms, and the firing of such live ammunition. 

    The Undersigned furthermore acknowledges that the Youth will be under constant adult supervision by Camp staff/volunteers and that The Instructor is not responsible for the conduct and behavior of the Youth. And any first aid or medical care is the responsibility of Camp staff/volunteers. 

    The Undersigned grants The Instructor permission to take whatever actions deemed necessary to assist the Youth during shooting activities or in the case of an emergency or unsafe situation, act immediately to preserve the safety of the Youth. This may include terminating shooting activities, momentary touching of Youth or taking control of a firearm while under the supervision of Camp staff. 

    This instrument binds The Undersigned and his/her executors, administrators, assignees or heirs.
