The following terms and conditions apply to avail of the services provided by J&M Safe Haven Enterprises, Inc., dba, Safe Haven Pet Resort (hereinafter referred to as “the Resort” By signing this Agreement, the Owner agrees he/she will comply with all applicable terms and conditions attached to this. If the Owner does not agree to any of the terms and conditions, he/she should not sign this Agreement.
The Resort reserves the right to refuse to accept any Pet(s) at check-in for any reason, including without limitation, if it appears Owner’s Pet(s) are sick, injured, in pain, or that its behavior could jeopardize the health, safety and well-being of our other Guests and employees.
1. Vaccination Requirements: Relevant vaccinations must be administered a minimum of 48 hours prior to arrival and 14 days prior for maximum benefits. Dogs: Rabies | DHLLP | *Bordetella (required every 6 months) | Cats: Rabies | FVRCP | *FeLV (*As always, please check with the Pet’s veterinarian as to what is best for the pet). All Guests, including puppies and kittens, must be 4 (four) months of age or older and received their full series of shots to enter the Resort unless cleared by their veterinarian. Owner understands and agree vaccinations do not cover all strains and it is still possible for Guests to contract a transmissible airborne, bacterial, or viral illness, such as, but not limited to; kennel cough, canine influenza, skin infections, papilloma virus, or other diseases wherever Pet(s) congregate such as parks, daycares, kennels, veterinarian's office and/or even the Owner’s neighbors’ pet(s) etc.
2. Illnesses, Injuries, and Pre‐existing Conditions: Owner represents their Pet(s) are in good health, free of fleas, worms, and have not been exposed to any contagious condition, illness or disease within the last 30 days that have not been disclosed to the Resort. Failure to disclose crucial information regarding any pre‐existing condition or contagious illness could be detrimental to the Pet(s) health as well as other Guest’s health and well‐being. Owner, hereby, are confirming all relevant information is shared and updated so we may care for the Pet(s) properly and keep all of our Guest’s safe as well. Owner is also confirming the Pet(s) have not had a new injury within the last 30 (Thirty) days that has not been disclosed to the Resort.
3. Daycare Requirements: Guests are required to pass a Behavior Evaluation ($16 per Guest fees apply). Guests cannot exhibit any aggressive behavior. All Guests must be spayed or neutered before they turn 6 (six) months of age to attend daycare. Private Suites are available for Guests that are unable to attend daycare.
4. Guest Behavior and Safety: Owner understands the risks involved with communal board of dogs and agrees to be solely responsible for any and all acts or behavior of the Pet(s) while in the care of the Resort. Owner confirms the Pet(s) do not have any behavior problems (including, but not limited to; toy, dog and/or people aggression, any type of biting behavior, reactive to any particular material(s). Owner agrees to be solely responsible for any and all acts or behavior of the Pet while in the care of the Resort, WHICH MAY INCLUDE PAYMENT OF DAMAGES FOR THE INJURY CAUSED BY THE PET(S) TO OTHER ANIMALS, HUMANS, AND/OR THE RESORT. IN ADDITION, OWNER AGREES THAT IF THE PET(S) IS INJURED BY ANOTHER PET, OWNER HEREBY RELEASE THE RESORT, ITS OWNERS, EMPLOYEES, AND AGENTS FROM ALL LIABILITY AND FINANCIAL RESPONSIBILITY FOR SUCH INJURY. OWNER FURTHER UNDERSTANDS THAT IF THE PET(S) BITES A HUMAN OR PET AND BECOMES DESTRUCTIVE OR HARMFUL TOWARDS ANYONE WITHIN THE RESORT PREMISES, THEN THE RESORT MAY CONFINE SUCH PET(S) AND/OR CONTACT THE APPROPRIATE AUTHORITIES AND/OR TAKE ANY OTHER NECESSARY MEASURE.
5. Food/Special Diet: To help prevent any added undue stress to our Guests digestive system, the Resort requires all Guests to continue their current diet at home. It is the Owner’s responsibility to ensure there is an adequate supply of food and/or medications for the Pet(s) entire stay while in the care of the Resort. In the event the Pet(s) run out of food, the Owner authorizes the Resort to purchase replacement supplies and agrees to reimburse the Resort for all associated costs, including, but not limited to food, medication, and transportation ($25 Taxi charges apply per occurrence).
6. Medications: Administered During Business Hours: 6am‐6pm ‐ $0 | After‐Hours: 6pm‐6am ‐ $65 per administration. Owner hereby gives the Resort permission to follow its own protocol (veterinarian‐approved) in the event the Pet(s) experience common ailments such as diarrhea, constipation and/or vomiting due to stress, also known as “Stress Colitis”. Owner also gives the Resort permission to add pumpkin/organic chicken broth and a special feeding protocol in necessary cases.
7. Personal Items: Clean bedding (blankets, towels, pillows, beds, etc.) will be provided for all Guests boarding overnight that are nondestructive. The Resort reserves the right to withhold any items that could be detrimental to our Guest’s safety, health, and well‐being. Please do not bring items that are valuable/priceless/ or irreplaceable as the Resort is not responsible for loss or damage to any personal items or toys left with the Pet(s).
8. Photo, Video, and Social Media Release: Owner grants and releases all rights to the Resort to own, take, edit, alter, copy, print, exhibit, publish, distribute, and make use of any and all photos or videos of my Pet(s) for any purposes without payment or any other consideration.
9. Veterinarian Release of Records, Liability, and Care: Owner hereby consents to the release of current vaccinations (including but not limited to the vaccines listed in clause 1 above) as well as any and all current and past medical records of the Pet(s) to the Resort, in the event past medical records are needed to verify pre‐existing conditions and/or for any other reason whatsoever. Owner authorizes the Resort to administer aid and/or obtain medical treatment for the Owner’s Pet(s) if he/she appears or becomes ill, injured, or exhibits such behavior that would suggest medical treatment is required. Owner permits the Resort to use the Owner’s veterinarian or any local or emergency veterinarian that is available at Owners sole expense for medical treatment, transportation, and/or any other costs for the purposes of such treatment. We require all vaccination records to be submitted to the Resort prior to arrival of the Pet(s). If the Pet(s) arrive with expired and/or no updated or verified vaccinations, Owner authorizes the Resort to transport Pet(s) immediately to the first available veterinarian to administer the updated vaccinations, provided, however, Owners local
veterinarian will be contacted first. For this purpose, Owner agrees to pay all costs and expenses incurred by the Resort.
10. Indemnification. For all Pet(s), Owner unequivocally indemnifies and holds harmless the Resort against any and all claims made against the Resort, its agents, respective officers, directors, employees, and any other agents, not otherwise listed. Owner states and agrees the Resort is not liable for any injuries or illnesses that occur as a result of the Pet(s) attendance, provided reasonable care and precautions are followed. Owner further understands and agrees the Resort is relying on Owner’s representations of the Pet(s) health and behavior. Owner understands in an emergency event, such as a natural disaster, every effort will be made to contract Owner to retrieve the Pet(s). Owner agrees the Resort, in its sole discretion, is authorized to transport and/or make temporary alternative arrangements to house and care for the Pet(s) until retrieved.
11. Release of Liability: Owner releases the Resort from any liability arising from Owner’s Pet(s) attendance at the Resort and fully accepts responsibility for any and all costs for injury to staff or other animals or damage to facilities caused by Owners Pet(s). The indemnification clause shall still apply even where due diligence and care have not been exercised.
12. Pick-up by Non-Owner: The Resort will not release any Pet(s) to any persons not listed on Owner’s contract unless prior arrangements have beenmade. In this case, non-owners will need to show a picture I.D. at pick-up. Please list all non-owners authorized to pick up the Pet(s)
