Submitting your expenses for the Chinook Winds Regional Gatherings requires completing the following steps.  There are two different ways you can submit your mileage and or expenses.

Option One:

Print & Mail

1. Fill out the following JotForm, then print the JotForm (print button is at located at the bottom of the page) and then mail in your completed Jotform, completed EFT form for approval to Chinook Winds Regional Council Address: P.O. Box 98 Okotoks, AB T1S 1A4.

Option Two:

Electronic Funds Transfer                                                   

2. Fill out the following JotForm, Register for Electronic Funds Transfer (EFT) by filling out the EFT form, upload your completed EFT form to the JotForm.  Payment for expenses will be made by Electronic Funds Transfer (EFT) and will take approximately 3 weeks for payment. If you have ever received EFT payment from the Region before you do not need to fill out the EFT form again unless your banking information has changed.

Mileage 

(Please enter the number of KM travelled, form will calculate amount to be claimed based on $0.40 a KM)
