The undersigned individual (the Member), hereby indicates their desire to become a member of The Smart Fit Method at (Location)Location* , pursuant to the terms and conditions of this gym membership agreement.

Trial Pass Expiration
The 'Trial Pass' expires 2 weeks after the purchase date.

Trial Pass Cost
Total cost for the 'Trial Pass' is $250.

Payment Methods
Credit card transaction (3.5% convenience fee will be added to each membership dues payment using this method)

Member Access
This gym membership agreement entitles the member to access to The Smart Fit Method so long as all dues are current. Hours and specific access are as follows:
- Personal training assistance for 3 x 20-minute prescribed workouts sessions per week during the following hours only : (i) Mondays to Fridays: 6am-7pm, (ii) Saturdays and Sundays: 8am-2pm.
- Workouts sessions include: (i) 1x SMART Cold HIIT (followed by 1x SMART Recovery sessions), (ii) 1x SMART Ride, (iii) 1x SMART Big 3/5, (iv) 1x SMART Body Scan consultation . Workouts by appointment only and should be booked in advance.
The Smart Fit Method reserves the right to demand additional fees for any additional workouts beyond the prescribed 3 workouts per week the Member chooses to engage in.
Health Attestation
The Member attests that they are fully able to participate in an exercise regimen without undue risk.
Booking Policies
The Smart Fit Method offers workout appointments on a first come, first served basis to its members. All workout appointments must be booked in The Smart Fit Method app. The Smart Fit Method does not guarantee specific availability to members and recommends members book in advance in order to secure their preferred appointment times.
- Appointments must be booked a minimum of 3 hours in advance. Last-minute requests for a workout appointment may be made by calling The Smart Fit Method during business hours.
- Appointments may be cancelled or rescheduled online within our booking platform up to 3 hours in advance. We do not accept cancellations by text, email, or phone.
- Workout appointments cancelled within 3 hours will be forfeited. If you cannot make your appointment, please cancel it.
- Body Consult scan appointments cancelled within 3 hours will be forfeited. If you cannot make your appointment, please cancel it.
Gym Rules & Participation
The Member hereby agrees to abide by all posted safety guidelines and regulations while using The Smart Fit Method facilities and equipment. Additionally, the Member agrees to dress and conduct themselves in a manner deemed appropriate for a fitness facility.
The Member shall not consume drugs, alcohol, or tobacco products on The Smart Fit Method property. The Member agrees not to photograph or videotape on The Smart Fit Method property. The Smart Fit Method reserves the right to revoke the Member’s access if these terms are violated.
Indemnification
This gym membership serves as a liability release, pursuant to which the Member agrees to indemnify and hold The Smart Fit Method harmless against any and all claims of loss or damage without limitation.

Lost Items
The Smart Fit Method assumes no responsibility for lost or stolen articles. Items not claimed within 7 days from our “Lost and Found” bin will be donated to charity.

Food and Drink
Only water in a closed container with a lid is allowed in The Smart Fit Method gyms.
Binding Law
This gym membership contract shall be considered binding upon the Member and The Smart Fit Method, and shall be upheld and enforced in accordance with the laws of (state) State*. Any legal proceedings related to this gym membership contract shall take place in courts located in (city) , (state)State* .

Photo Release
I hereby grant the The Smart Fit Method permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the The Smart Fit Method and will not be returned.
I hereby irrevocably authorize the The Smart Fit Method to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the The Smart Fit Method from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Acceptance
The below signed party/individual hereby enters this membership contract with The Smart Fit Method acknowledgement and acceptance of the terms listed above.
