I will attend the Let’s Talk Mother/Daughter Conversation, sponsored by Black Girl Rising, Inc. (BGR), on Sunday, May 22, 2022, from 2:30 pm – 4:30 pm at the Martin DePorres Center at St. Mary of the Spring, 2230 Airport Drive, Columbus, OH 43219.

I understand that BGR and its partners and sponsors are not liable for illness, accidents, or injuries that may occur at the event. Suppose I cannot be reached to make arrangements for emergency medical care at the time of illness, accident, or injury. In that case, I authorize BGR to take my child to the nearest hospital with emergency care.

The BGR Covid‐19 policy recommends mask‐wearing. Masks should be worn covering both the nose and mouth. Fresh masks will be provided for all attendees. Hand sanitation products will be available, and attendees are requested to sanitize their hands frequently. Where possible, refreshments will be individually wrapped/boxed with no shared utensils.

I grant BGR and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and video images taken of me, for publication, promotion, illustration, advertising, or trade, in any manner or any medium. I hereby release BGR and its legal representatives for all claims and liability relating to said images or video. Furthermore. I grant permission to use any of my comments or statements provided during program sessions, with or without my name, for advertising and publicity without restriction. I waive my right to any compensation.
