NE and I hereby grant:
- Northeast Nebraska Community Action Partnership (NENCAP), State, and Federal Personnel permission to inspectmy home.
- NENCAP, permission to weatherize in accordance with programregulations
- I will allow my home to be photographed for pre-weatherization and post-weatherization documentation.
- I understand that all weatherization work completed by NENCAP will be at no cost to me and that no lien or debt will beincurred.
- I understand that all information will be kept confidential and not be made public in a manner that will identify the dwelling or the occupants.
- I intend to reside at the property to be weatherized for at least one year.
- I understand that interior window/door trim and wall patches will be installed to a pre-paint/finish stage.
- I will agree to any procedures necessary to insulate the sidewalls
- I will allow program personnel to apply caulk to the interior of my home. I understand that some types of caulking may emit nontoxic odors for up to 2 weeks after application.
- I understand that my application is valid for 12 months from the date it was signed and that it may require recertification after that time in order for my home to be considered eligible for weatherization assistance.
Any persons in the household with respiratory problems, pregnancy, or any other medical condition which could be aggravated by cellulose insulation dust, caulking fumes, etc. should consult a physician prior to weatherization work.

- If NO, I agree to allow entry during my absence
- I agree to remove all valuable items from exterior walls and crew work areas to prevent breakage.
- I agree to maintain responsibility for the safety of minor children during the construction process.
- I agree to keep all pets restrained and away from the work area and crew.
I understand that even properly installed materials can sometimes cause problems, such as:
- Doors and windows increasing and decreasing in size because of humidity and temperature changes.
- Moisture developing on primary and storm windows
- Attic vents allowing moisture entry during severe windstorms
I understand that the upkeep of materials installed is the responsibility of the homeowner.
