This form is for filings in the counties of Albany, Schoharie, Saratoga and Schenectady.

New York City residents filing through a registered agent service must file from this form.

Your affidavit will be mailed out within 7 days of ad completion. Keep in mind all LLC ads must run for 6 weeks. We cannot mail affidavits prior to completion. We are not responsible for slow or non-delivery of the USPS. Please note there will be a $25 charge for resending an affidavit so to avoid this It is your responsibility to give us the address and or department in which you receive your mail. Thank you.
