A non-refundable research fee, based on your group size, is required to begin. This fee does not apply toward the final balance of your cruise package. ⚠️ IMPORTANT TRAVEL DOCUMENTATION NOTICE

Passports: Ensure your U.S. passport is valid for at least six months beyond your planned return date. Many countries enforce this rule, and airlines may deny boarding if your passport expires sooner. It's advisable to renew your passport at least nine months before expiration to avoid travel disruptions.

REAL ID Requirement (Effective May 7, 2025): To board domestic flights or access certain federal facilities, U.S. travelers aged 18 and over must present a REAL ID-compliant driver's license or ID, identifiable by a star symbol in the upper right corner. Alternatively, a valid U.S. passport, passport card, or other TSA-approved identification is acceptable. Standard state IDs without the REAL ID designation will no longer be accepted for these purposes.

For your protection, Travel Insurance is strongly recommended and available upon request from Forever A'DAE Travels. We strongly recommend purchasing travel insurance for your trip. Unexpected events can arise before or during travel—such as trip cancellations, medical emergencies, lost luggage, or travel delays—and travel insurance offers valuable protection and peace of mind. Without it, you may be responsible for significant out-of-pocket expenses. Travel insurance not only safeguards your investment but also ensures access to emergency assistance services should anything go wrong while you're away. For your security and convenience, we highly encourage you to consider this essential coverage.

Forever A'DAE Travels does not act as an insurance agent and cannot sell or guarantee coverage. However, we can refer you to a trusted provider and assist you in the purchase process.

Final travel documents (e.g., tickets, vouchers, itineraries) will not be released until this form is completed and signed.

To decline recommended travel insurance, your signature on this insurance waiver form is required. Final travel documents (tickets, vouchers, etc.) cannot be sent to you prior to receipt of the signed insurance waiver
