Discipline Statement*

A discipline statement describes your artistic practice. It can be as simple as “This is a film production shot on 16mm film”, "We teach dance classes to youth" or gives a more nuanced explanation of your practice, such as, "Our artist collective works with grassroots organizers to create and document culturally relevant murals throughout King County."

Project Timeline*

Please provide a timeline for the project being applied for, or in the case of organizations applying, a timeline of organizational plans ahead, anything that will give further context to the planning of programs being offered. Rough timelines are very acceptable, this field is just to get a sense of scope of organizational needs or project deadlines ahead.

Biography*

Please provide a short biography. For artist teams/arts collectives, submit a bio for the project lead.

Budget Attachment*

Please provide any budgetary documents for the project or organization that you have. These do not have to be exact to the cent, but we must have a budget document on file to establish sense of financial scope and where donated funds will be allocated. And Excel spreadsheet is preferred, but any PDF or similar document detailing financial breakdown is also acceptable.
