Consent form is valid for 12 months. If any changes have occurred within 12 months since form has been submitted, please fill out new consent form prior to appointment and notify your spray tan artist.

I have read the spray tanning preparation and aftercare guidelines on bbtanning831.com and understand that not following these guidelines will affect the result of my services with Bronzed Babe Airbrush Tanning
*NEW POLICY STARTING JANUARY 2024* Four consecutive cancelled appointments in a row will result in future appointments requiring a non refundable $25 deposit. This also includes appointments, rescheduled for different days (excluding appointments that are rescheduled for a different time on the previously booked day).
*NEW GROUP POLICY STARTING JANUARY 2024* groups of 4 or more are subject to a 48 hour cancellation policy. Any members of the group are subject to the no show/late cancellation fee depending on the time and nature of the cancellation. A non refundable $100 deposit may apply to groups and is subject to change based on the size of the group. Custom group pricing can be quoted by the owner only and is subject to change if the final number of group is altered. Groups of 4 or more shall not be split payment or paid individually, change in full will be applied to the card on file provided by the group organizer along with the non refundable deposit. An automatic 20% gratuity will be applied to all groups of 4 or more
Have you had a sunburn in the last 4 weeks. (tanning over sunburned skin or previously burned skin that has peeled, will not be guaranteed by Bronzed Babe Airbrush Tanning and will likely produce an unfavorable result)
I understand that services performed by Bronzed Babe Airbrush Tanning are not "results guaranteed" and may vary by person. This may include: medical conditions, skin type, skin texture, absorbency abilities, spray tan prep, and aftercare.
The use of spray tanning or self tanning is a skin-safe practice; which can not absorb any further than the first, topical layer of skin. There is no evidence that these ingredients penetrate the blood stream or hair follicles. The FDA has approved sunless tanning for external use only. I understand that product being applied to my skin during a spray tan service performed by Bronzed Babe Airbrush Tanning is temporary. Spray tanning solution does not contain sunscreen or U.V. production. I understand that I must continue to wear sunscreen for skin protection.
During my spray tan with Bronzed Babe Airbrush Tanning, I understand that cosmetic staining agents might be applied to my skin. I understand that this may be transferable to clothing, fabrics, materials, or other persons but are easily removable/machine-washable from most garments. Bronzed Babe Tanning is not responsible for any transfer of these cosmetic bronzers.
Bronzed Babe Airbrush Tanning will NOT issue refunds due to dissatisfaction caused by lack of preparation, failure to follow instructions or negligence from clients. If an issue with my service has taken place, I understand that I must contact Bronzed Babe Airbrush Tanning within 48 hours of my appointment. I understand that resprays and complimentary touch ups are at the discretion of Bronzed Babe Airbrush Tanning only.
I understand that due to sanitary purposes, any retail products purchased from Bronzed Babe Airbrush Tanning are non-returnable and non-refundable.
I understand the Bronzed Babe Airbrush Tanning has a 24 hour cancellation policy, I understand that I will be subject to pay cancellation fees if I do not cancel my appointment prior to this time frame. I understand that showing up more than 5 minutes late to my scheduled appointment will result in a charge of 95% as a "no show" and is not transferrable or refundable. I understand any mobile appointments require a $40 non refundable deposit
Consent Agreement
I affirm that I'm fit and certified to go on with my requested procedure by Bronzed Babe Airbrush Tanning. I confirm that I do not possess any medical conditions to the best of my knowledge that prohibits me to complete the procedure. I affirm that I have not previously had any spray tan services or used any self tanning products that resulted in an allergic reaction.

I understand that I am compelting this service at my own risk and I hearby hold Bronzed Babe Airbrush Tanning harmless from any actions, judgments, demands, liability, loss, theft, adverse reactions, injury to persons, damages to property, clothing or personal belongings as a direct result of services acquired.
