Cancellation Policy
A non refundable 10% booking fee is required for booking confirmation. Full payment is required 72 hours before the service is provided. If cancellation occurs within 72 hours of the service start time the full payment will be reatined.

Vet Release Form
During my absence, Wrexham Pet Care will be caring for my pet(s). In the event of an emergency, I authorise you (veterinarian) to administer medical treatment and will be responsible for payment to you (veterinarian) upon my return.

I,   First Name*   Last Name*  give Wrexham Pet Care permission to transport my pet(s) to the above veterinarian and authorize treatment in the event of an emergency or sickness.

If this veterinarian is not available, I authorise Wrexham Pet Care to transport my pet(s) to a veterinarian of choice and authorise treatment. If emergency care is needed after regular office hours, my pet(s) may be taken to the nearest Veterinarian Emergency Clinic/Hospital.

I give permission to Wrexham Pet Care to approve treatment up to £   how much*    . I agree to be responsible for all charges upon my return including, but not limited to, vet fees, extra visit fees and transportation fees.

This release will remain valid for all current and future visits unless a new release is signed.
Key Release Form
Will we require your house keys?*
I,   First Name   Last Name (hereinafter referred to as “client”), agrees to give Wrexham Pet Care a key to my home located at     Address

Wrexham Pet Care will safeguard Client’s key(s) in a manner consistent with that of the professional pet/home service industry, which includes tagging, coding and storing of key(s) in a manner that offers reasonable protection to Client in the event of loss or theft of key(s). Following the service period, Wrexham Pet Care will securely retain Client’s key(s) for safekeeping until Client contacts Wrexham Pet Care to arrange repossession of key(s).

IMPORTANT: Wrexham Pet Care will not agree to leave your house key on a counter in your home on our last visit due to possible return delays where your home and pets would be left neglected. The safety of your home and pets are our top priority while you are away. This policy will give peace of mind to both Wrexham Pet Care and Client should arrival home be later than expected.

Key Return

There is no charge to return your house key; however, we will only return key(s) to the Client or Client’s representative. (Upon request to   First Name   Last Name if Client is not available, Client authorises   First Name   Last Name to accept the key.)

Mixed Animal Disclaimer
Are you happy for your dogs to be mixed with others?
I, First Name Last Name , give the Wrexham Pet Care Provider permission to walk my dog with other dogs.

I understand that all other clients have also agreed to mixed boarding and that this declaration will be available at all times for review by the licensing authority.

I freely and voluntarily enter into this contract of permission with the Wrexham Pet Care and fully understand the above conditions.

This permission can be part of the Service Agreement but cannot replace the Service Agreement in part or in full.
Off-Lead Disclaimer
Can your dogs walk off lead
I understand that I, First Name Last Name am solely responsible for any harm caused by my dog(s) while my dog(s) is/are staying at Wrexham Pet Care. I hereby give Wrexham Pet Care permission to take my dog on off-lead walks in trust that this will be done in a safe area away from traffic.

I certify that, to my understanding, my dog is trustworthy off lead and has good recall. I realise that there are certain risks involved with off lead walks and that I will not hold Wrexham Pet Care responsible in any way should any harm befall my dog. I further understand that due to the way that dogs interact with one another, minor cuts and scratches can occur even though the dogs are carefully supervised at all times.

While my dog(s) is/are in the care and custody of Wrexham Pet Care, if I am unreachable in the event of an emergency, I hereby authorize Wrexham Pet Care, its agents, and/or representatives to seek immediate veterinary care for my dog. I understand that all costs in connection with, veterinary, medical or other treatment, shall be my responsibility.

I hereby release and agree to save and hold harmless, Wrexham Pet Care, it's employees, assistants and agents from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify Wrexham Pet Care for any and all such liability, claims, suits, actions, losses, injury or damage. I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement.

I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of the Booking form completed by me. Although we carefully screen all applicants, to protect all dogs in our care, we may (very occasionally) need to separate a dog that displays aggressive behaviour. Wrexham Pet Care reserves the right to relocate a dog if it proves necessary
Payment
Upon completition of this form you will receive an invoice for a 10% deposit. This will be required before your booking is confirmed.

Payment must be provided 72 hours prior to the service start time. Failure to do so will lead to service cancellation.

We cannot guarantee the health of any animal, but we pledge to provide appropriate care to all boarders. I agree to hold this facility harmless for conditions that often are unavoidable in boarding environments, including, but not limited to, weight loss or gain, rough hair coat, kennel cough, upper respiratory infection, diarrhea, and fleas.

If pets are boarding in the same enclosure we reserve the right to separate if problems arise.
