Registration: Open *Must have played Association U17/15-A1 or higher in 2021/22

A consent form must be filled out for each camp/clinic a player is registered in. If registering for multiple camps/clinics, separate consent forms must be filled out each time of registration. In some instances, a third-party consent form will also be required at the time of registration or in a pre-camp/clinic email.

Covid-19 remains a worldwide pandemic. To maximize the safety under the current covid-19 conditions staff and participants are requested to follow the guidance published by Federal & Provincial health authorities, the Government of BC, and Worksafe BC. Please be aware that third-party may choose to adopt more rigorous policies or procedures for the facility we run our programs.

You are required to read and confirm that you understand the provided/third-party covid-19 safety plan.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Euro Elite Hockey has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Euro Elite Hockey can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
I voluntarily seek services provided by Euro Elite Hockey and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Euro Elite Hockey harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Euro Elite Hockey. I understand that this release discharges Euro Elite Hockey from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Euro Elite Hockey. This liability waiver and release extends to the salon together with all owners, partners, and employees

Consent and Liability Waiver

In consideration of the 2022 Euro Elite Hockey allowing me to participate, I agree and understand the following:

Euro Elite Hockey reserves the right to reschedule or regroup players according to enrolment and availability. Euro Elite Hockey*, the hockey complex* and employees will not be liable or held responsible for any accidents or loss however caused and agreed to release all mentioned parties from all claims and damages. I further verify that my child, my ward, or I have no medical problems and is in good physical health and the above-mentioned parties will not be held responsible for any medical, dental, or insurance claims resulting from the injury.

Cancellation Policy

Planning for all Euro Elite Hockey camps and programs is based on current enrolment numbers well in advance of the camp/program start date. As a result, no refunds or cancellations are permitted, except for medical reasons. If a player is unable to attend the camp/program (and notice is given prior to the start date) due to medical reasons, he/she will receive a credit for their full amount paid less a $30 administration fee, which may be used for any camp/program taking place within the next one (1) full calendar year. A doctor’s note is required to substantiate this type of cancellation, and this note must be provided within 1 week of notification. Please also note that credits issued are non-transferable. We thank you very much for your understanding and support of our programs.

Camps Cancelled due to COVID-19

Any person registered in a Euro Elite Hockey Camp that is canceled by Euro Elite Hockey due to COVID -19 will receive a FULL Credit for future Camps.
