COVID Policy | Release of Liability | Permission to Secure Treatment
We urge students to attend classes regularly. MASKS ARE OPTIONAL FOR ALL (Students, Tutors, Parents, Visitors). However, if at anytime the student - or a member of the student's household begins experiencing a cold, fever, runny nose, cough or any symptoms associated with COVID-19, we ask that you remain home and not present in class for 1 week, or until symptoms subside. I agree to indemnify and hold harmless and defend M.O.R.E of Baltimore Homeschool and its officers, agents, volunteers and employees from any and all claims resulting from injuries, damages, and losses sustained by me or my minor
child arising out of, connected with, or in any way associated with the activities of the program(s).
In the event of any emergency, I authorize M.O.R.E of Baltimore Homeschool to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for me or my minor child's immediate care and
agree that I will be responsible for payment of any and all medical services rendered.
By signing below, I acknowledge the agreement stated in this form. I also confirm that all information I entered in this form is true and accurate.
Furthermore, I understand that M.O.R.E of Baltimore: Homeschool Co-op was designed to supplement, not replace, my homeschool instruction. As the parent, I am enrolling my child in the tutorial with the understanding that I am ultimately responsible for providing regular, thorough instruction in english, math, science, social studies, art, music, health, and PE; while maintaining a portfolio of my child's work for review by my local district reviewer or umbrella program.
