Information from sacramental records is only released to the individual who received the sacrament, the parents of a minor (currently under the age of 18), or a requesting Catholic Church institution (e.g. another parish or tribunal). You will also need to provide a copy of a government-issued photo ID (e.g. Driver's License or passport). Please allow up to 2 weeks for processing. Incomplete forms will delay the processing of your request.
