Tomando en consideración los servicios de Puerto Rico Alliance for Companion Animals, Inc., sus empleados, directores, agentes, oficiales, voluntarios, participantes y toda otra persona o entidades que actúe en cualquier capacidad a favor de Puerto Rico Alliance for Companion Animals, Inc., y en contraprestación váliday onerosa, por la presente relevo y eximo a Puerto Rico Alliance for Companion Animals, Inc. de cualquier responsabilidad que surja por negligencia, en mi nombre, a nombre de mi esposa/o/pareja, de mis hijos, mis padres, mis herederos, personas designadas, representante personal de mi sucesión como sigue:

1. Certifico que tengo un seguro adecuado para cubrir cualquier lesión o daño que pueda causar o sufrir mientras participo, o estar de acuerdo con pagar los costos de tal lesión o daño que sufra yo mismo(a).
2. Entiendo que podría ser fotografiado(a) durante actividades como voluntario(a) le concedo a Puerto Rico Alliance for Companion Animals, Inc. el derecho absoluto e irrevocable, al igual que permiso no restringido, con respecto a cualquier fotografia que la organización me haya tomado o pueda tomar, o en la cual yo pueda aparecer acompañado(a) de otras personas, para usarlas, reusarlas, publicarlas o volver a publicar las fotografías, ya sea una parte de la misma o en su totalidad, individualmente o junto con otro material, en cualesquiera o todo medio ahora o en el futuro, incluido el internet; para cualquier propósito que fuese, sin restricciones con respecto a su alteración; y a utilizar mi nombre en cuanto a cualquier uso, si así lo decidiesen. Relevo y eximo de cualesquiera y toda responsabilidad a Puerto Rico Alliance for Companion Animals, Inc. de cualquier reclamo o demanda que pueda surgir relacionadas con el uso de las fotografías, incluido sin limitación alguna de cualesquiera y todo reclamo por difamación o violaciones de cualesquiera derechos de publicidad o privacidad. Este relevo será vinculante para mí, mis herederos, representantes legales y personas asignadas.
3. Estoy de acuerdo en que, si cualquier porción de este acuerdo se considerase nula o no ejecutable, la porción restante permanecerá en vigencia y efecto plenos.
4. Al firmar este documento acepto que, si alguien se lesiona o si se dañase alguna propiedad durante mi participación en esta actividad, en una corte de ley podrían encontrar que he renunciado a mi derecho a mantener una demanda en contra de las partes, a base de cualquier reclamo por negligencia de los cuales los relevo en este documento. He tenido suficiente tiempo para leer este documento en su totalidad, entiendo su contenido y acepto estar sujeto sus términos. Certifico que las declaraciones hechas en esta solicitud para voluntarios son legítimas y verdaderas y que han sido expuestas voluntariamente. Entiendo que esta información se mantendrá en la mas estricta confidencialidad, y relevo a Puerto Rico Alliance for Companion Animals, Inc., las tiendas PetSmart y PetSmart Charities de cualquier responsabilidad por suplir tal información. También entiendo que no se me pagará por mis servicios como voluntario(a) de modo alguno, y que cualquier gasto incurrido por mi mientras actúo como voluntario(a) no me será rembolsado sin que antes haya sido aprobado por la junta de directores. In consideration of the services of de Puerto Rico Alliance for Companion Animals, Inc., its employees, directors, agents, officers, volunteers, participants, and all other persons or entities acting in any capacity on their behalf, and for other good and valuable consideration, hereby agree to release and discharge Puerto Rico Alliance for Companion Animals, Inc., from liability arising from negligence, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estateas follows:

1. Certify that have adequate insurance to cover any injury or damage may cause or suffer while participating, or else agree to bear the costs of such injury or damage to myself.
2. I understand that may be photographed during volunteer activities and grant Puerto Rico Alliance for Companion Animals, Inc., the absolute and irrevocable right and unrestricted permission concerning any photographs that the organization has taken or may take of me or in which may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, without restriction as to alteration; and to use my name in connection with any use if he/she so chooses. release and discharge Puerto Rico Alliance for Companion Animals, Inc., from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This release shall be binding upon me and my heirs, legal representatives, and assigns.
3. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portion shall remain in full force and effect.

By signing this document, agree that if anyone is hurt or property is damaged during my participation in this activity, may be found by court of law to have waived my right to maintain a lawsuit against the parties being released on the basis of any claim for negligence from which have released them herein. have had sufficient time to read this entire document, understand its contents, and agree to be bound by its terms. - certify that the statements made in this volunteer application are true and have been given voluntarily. Understand that this information will be held in the strictest confidence, and release Puerto Rico Alliance for Companion Animals, Inc., PetSmart Stores and PetSmart Charities from any liability whatsoever for supplying such information. I also understand that will not be paid for my services as a volunteer and that any expenses incurred while volunteering will not be reimbursed without prior approval from the board of directors. Relevo de menores: Soy el padre/representante legal del voluntario nombrado en el campo de "Nombre/Name", quien es menor de 18 años de edad. Al firmar este relevo acepto el Consentimiento, relevo de responsabilidad y acuerdo de no responsabilidad aquí incluído a nombre de mi hijo(a), y acepto tomar responsabilidad total del comportamiento de mi hijo(a) mientras actúe como voluntario de Puerto Rico Alliance for Companion Animals, Inc.

Minor Release: I am the parent/legal guardian of the volunteer named in the field "Nombre/Name", who is under the age of 18 years old. By signing this waiver agree to the above Consent, Release and Hold Harmless Agreement on behalf of my child and agree to take full responsibility for my child's behavior and actions while volunteering with Puerto Rico Alliance for Companion Animals, Inc.
