Birthday Party Agreement
Birthday party is $475 for up to 15 children. Each additional child is $25 per child. This completed contract, along with a non-refundable deposit of $100 is required to secure a booking. Birthday parties are only booked at least 2 weeks in advanced of event date. We don't accept day of bookings. Your date is not confirmed until you are contacted by Forever Dancing staff to verify receipt and availability. Dance portion is the first hour of your party and food is last 30min. Included for up to 15 children: 2 party rooms, 1 pizza slice per child, one bottle of water per child, paper goods, table & chairs, set up/clean up. Host must bring their own non-frozen cake or cupcakes. No other outside food is permitted. Location: 214-10 41st Ave Bayside, NY 11361
