I agree to release and indemnify The Fitness Centre, Plus Fitness Glenmore Park located at shop 1c, 1-11 Town Terrace, Glenmore Park, NSW 2745, as follows.

1. I Participate in the activity at my sole risk and responsibility.

2. I release and hold harmless the Fitness Centre Operator (Plus Fitness), its servants and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for or in respect of or arising out of injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever.

3. I accept all responsibility for any injury, loss or illness that may arise from use of the centre's equipment, or any other product used within the centre. I will not hold the centre responsible in any way.

I also agree that, In the event that I am injured, or my property is damaged, I will bring no legal claim or otherwise, against The Fitness Centre Operators in respect of that injury or damage.

Before signing this register or document I have read and understood it and know that it affects my legal rights.

Where the participant is under 16 years of age. I being the parent or guardian of the person named in this acknowledgement and release hereby acknowledge and agree;

1. I have read the whole of this document and understand it.

2. I consent to the person named in this acknowledgement and release participating in the activity and I am aware of the risks, dangers and obligations set out above in this acknowledgement and release Plus Fitness.

I give full consent and authority for the abovementioned to attend the gym.

I am aware that by signing this form this affects my legal responsibilities. In consideration of payment for participating in the activity (and except to the extent that the same may be precluded by statue).

I agree to release and indemnify Plus Fitness and its Operators, Owners, Franchisees, Staff, Contractors and Franchisor as follows;

1.The abovementioned participates in the activities in the gym at their and my, own risk and responsibility.

2.I release, and hold harmless Plus Fitness, it’s servants and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for or in respect of, or arising out of injury, loss, damage or death caused to my child, my child’s or my property whether by negligence, breach of contract or in any way whatsoever.

3.I accept all responsibility for any injury, loss or illness that may arise from my child’s use of the gym, classes or any other product or service used within the gym. I will not hold the gym responsible in any way. I also agree that in the event that I or any children in my custody or on the premises of the gym are injured or my property is damaged, I will bring no legal claim or otherwise against Plus Fitness, it’s servants and agents in respect of that injury or damage.

Photo/Video Release I hereby grant the Fitness Centre (Plus Fitness Glenmore Park permission to use my my or if under 16 years of age, my childs likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web- based publications, without payment or other consideration. I understand and agree that all photos will become the property of Plus Fitness Glenmore Park and will not be returned. I hereby irrevocably authorize Plus Fitness Glenmore Park to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my or if under 16 years of age, my childs likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Plus Fitness Glenmore Park from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate or on behalf of my child have or may have by reason of this authorisation. I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE.
