Bow Wow Buddies Foundation Grant Application

This application is for individuals or families with a dog in need of life-saving veterinary care.

Grants are not available for preventative care such as spay/neuter, dental or procedures, or vaccines; for preventable diseases such as heartworm, pyometra, or dental disease; and for chronic diseases such as Cushings, Addisons, IMHA, or diabetes amongst other conditions. Very limited grants may be available for orthopedic issues such as ACL, CCL, FHO, or others.

Before you apply, you must read and acknowledge the following:

I understand that I am applying for a grant on behalf of my dog and that funds are not guaranteed no matter the need or circumstance. Any inappropriate or threatening contact will result in the being ineligible to apply for future grants.

To the best of your ability, please select what illness or injury your dog is suffering from and for which you are requesting a grant to treat.

Diabetes, IMHA (immune disease), glaucoma, Addison's disease, Cushings, kidney disease, or other, chronic illness

Cancer

Orthopedic injury/issue (torn CCL, torn ACL, hip dysplasia, IVDD, luxating patella, etc.)

Parvovirus, distemper, canine influenza, hepatitis, or leptospirosis

Dental disease (tooth decay, gum inflammation, plaque buildup, tooth fracture)

Spay or neuter (including pyometra and retained testicles)

Broken limb or fracture

Heartworm, hookworm, giardia, or other parasite

Multiple injuries from accident

Bloat, gastroenteritis, or pancreatitis

Intestinal obstruction

Poisoning or toxicity

Unknown

Upload Estimate of Treatment Costs from Your VeterinarianThis is REQUIRED. If you do not submit an estimate or invoice from a veterinary facility, you will not be considered for a grant. Only costs associated with the proposed treatment will be considered during review. Estimates must include total cost, a breakdown of the different treatments used, and must show the full veterinary hospital name and address. Estimates need not be for the specific dog, but must show proposed treatments and cost.
