Permission is hereby granted for my child to accompany you on the Youth Choir
Tour, June 16 - 24, 2023. In consideration for your taking my child for the week, I
understand and agree that from the time my child leaves for Youth Choir Tour 2023
until his/her return every day he/she will be under your control and supervision and
under the control and supervision of those assisting you. You have my permission to
exercise parental supervision over him/her in respect to appropriate behaviors within
the guidelines of the Church.
