You must submit this form with all presenter information in order for your abstract to be considered for presentation in the SOPHE 2022 Advocacy Summit. As a reminder, all presenters and co-presenters must register for the full conference by Friday, August 26 . If you do not register by August 26, 2022, SOPHE reserves the right to modify the program and re-assign your spot to another presenter. If you plan to have more than 4 total presenters, please email advocacy@sophe.org. If you have any other questions, please email advocacy@sophe.org
