This form will allow you to report all the ways you have earned credits. Please fill out one form per month.
- Credits will be evaluated at the end of each month and uploaded by the end of the ensuing month.
- e.g. June activities should be reported by the first week of July, and Selkirk will evaluate and upload credits by the end of July.
- Activities over 2 months old may not be reported and will not be accepted.
- Credit will not be given for activities prior to May 1, 2022, the start of this program.
***To ensure credit is received for qualifying activities, be sure to answer all questions related to the activity for which you are seeking credit.
Please note: We will approve only one submission per month; any additional submissions within the same month will be rejected, and no credits will be awarded. To facilitate processing, consolidate all activities into a single submission.
If proper documentation is not received, then credit will not be given. Unfortunately, due to the volume of submissions, we are unable to reach out when proper documentation is not provided.
