A. I, First Name*   Last Name*, recognize that the use of the equipment and facilities of the Mt. Diablo Unified School District ("MDUSD") while participating in the activities of the Walnut Creek Wrestling Club ("WCWC") involves a risk in the physical injury including that caused by my negligence or the negligence of WCWC, its agents, representatives, and employees. I hereby agree to assume the risk of the injury in its entirety regardless of cause.

B. I hereby voluntarily and forever release, discharge, waive and relinquish any and all actions, causes of action, or claims, including claims for personal injury, property damage or wrongful death, occurring to me, against WCWC, MDUSD, and their agents, representatives and employees ("Released Parties") in connection with or arising out of my use the MDUSD equipment and facilities. I further relinquish any action, causes, of action, or claims which may be hereafter arise, and agree that under no circumstances will I present any claim, including any claim for personal injury, property damage or wrongful death, against the Released Parties, in connection with or arising out of my use of the MDUSD equipment and facilities.

C. I agree that in the event any claim or cause of action in connection with or arising out of my use of the MDUSD equipment and facilities, including any claim for personal injury, property damage or wrongful death to the undersigned, is brought against the Released Parties, I shall indemnify and hold harmless the Released Parties from any and all such claims and causes of action.

D. BY SIGNING BELOW, IT IS THE INTENTION OF THE UNDERSIGNED TO EXEMPT AND RELIEVE THE RELEASED PARTIES FROM ANY AND ALL LIABILITY IN CONNECTION WITH OR ARISING OUT OF HIS OR HER USE OF THE MDUSD EQUIPMENT AND FACILITIES.
