  • We, the SuperDive Team, pledge the following:

    • Commitment to Inclusivity & Integrity
      • We pledge to provide an intentionally welcoming environment, safe from harassment, for all.

      • We take safety extremely seriously and pledge not to cut corners. Safety will be the ultimate deciding factor in our decision-making. 
      • We promise to put our students' best interests ahead of making a quick buck at their expense.
    • Staff Policies
      • In an effort to bring attention to unfair labor conditions in the dive industry, we declare that all of our staff - including Divemasters - are compensated no less than the minimum living wage of their locale. 
      • Though appreciated, we do not expect our customers to provide monetary tips. However,  SuperDive NYC does award monetary bonuses based on post-session feedback from customers.
      • Our active SuperDive Professionals are subject to an annual review of skills and fitness.
    • Insurance
      • DAN Student Medical Coverage Insurance policies are included with every entry-level certification course and are valid until completion of training.
      • For everyone else, we recommend purchasing Diving Insurance through DAN (Divers Alert Network), as well as appropriate travel insurance for dive travel. No pressure, but we'd be grateful if you used our affiliate link.

    As a participant, you understand and agree that:

    • Refunds and Rescheduling
      • Cancellations made within 72 hours of an appointment will be subject to a rebooking fee of $40. No-shows or cancellations within 24 hours of an appointment will be subject to a rebooking fee of 40% of the session value. These fees may be waived at the discretion of SuperDive management in the case of a documented emergency or other force majeure. 
      • Itineraries and schedules are subject to modification due to availability of third-party vendors, weather conditions, or other circumstances out of SuperDive NYC's control.
    • Additional Fees 
      • We don't believe in sneaky hidden fees! However, be aware that sometimes, especially when traveling for Open Water sessions, you may have to plan for additional expenses associated with: dive site admission, travel, overnight stays, and/or equipment rental (if not using SuperDive's equipment), etc. 
    • Medical Questionnaire
      • You must answer each question on the Medical Questionnaire honestly with a YES or NO. If you are unsure, answer YES. Staff of SuperDive cannot provide medical advice. 
      • After filling out the questionnaire, you may be told that you require a Medical Evaluation before in-person training. You cannot attend in-person training until obtaining a doctor's approval - no exceptions.
      • Should you experience a medical condition or concern during your training that would change your answers on the Medical Questionnaire, we are required to suspend your training until you complete a new Medical Questionnaire and obtain a doctor's approval to continue training. 
      • A medical clearance does not constitute a mandate to an instructor to train an individual. The diving instructor, who observes the candidate in the field, may also decide to discourage an individual from diving based on their assessment of that individual's suitability for diving.
      • Participants, even if they are licensed doctors, may not sign off on their own medical questionnaires. 
    • Attitude Agreement
      • You will uphold our principles of an inclusive environment and act with respect towards staff and/or other students and participants. 
      • You will communicate and navigate any issues with SuperDive staff with a cooperative, empathetic, and problem-solving attitude.

  • Diver Medical Participant Questionnaire

    Recreational scuba diving and freediving requires good physical and mental health. There are a few medical conditions which can be hazardous while diving, listed below. Those who have, or are predisposed to, any of these conditions, should be evaluated by a physician. This Diver Medical Participant Questionnaire provides a basis to determine if you should seek out that evaluation. If you have any concerns about your diving fitness not represented on this form, consult with your physician before diving. If you are feeling ill, avoid diving. If you think you may have a contagious disease, protect yourself and others by not participating in dive training and/or dive activities. References to “diving” on this form encompass both recreational scuba diving and freediving. This form is principally designed as an initial medical screen for new divers, but is also appropriate for divers taking continuing education. For your safety, and that of others who may dive with you, answer all questions honestly.

    Complete this questionnaire as a prerequisite to a recreational scuba diving or freediving course.

    Note to women: If you are pregnant, or attempting to become pregnant, do not dive.

    Please take your time to go carefully go through the six agreements. Diving and dive-related travel are inherently risky so this form may take several minutes to complete. This form is legally binding; please be certain you understand the implications of signing. Please only sign each section after reading it carefully and agreeing completely. At the end of the document, please have a Witness (anyone of legal age and competent to sign) affix their signature.

  • I do hereby affirm and acknowledge that I have been fully informed of the inherent hazards and risks associated with freediving, mermaiding, snorkeling, skin and/or scuba diving, and instruction related thereto (“Diving Activities”).

    I understand that these risks can lead to severe injury and even loss of life. I understand that diving with compressed air involves certain inherent risks; including but not limited to decompression sickness, embolism or other hyperbaric/air expansion injury that require treatment in a recompression chamber.

    I understand that freediving, mermaiding, snorkeling, skin diving, and scuba diving are physically strenuous activities and that I will be exerting myself during Diving Activities, and that if I am injured as a result of heart attack, stroke, panic, hyperventilation, drowning or any other cause, that I expressly ASSUME THE RISK of said injuries and that I will not seek to hold any other partyresponsible for the same.

    I understand that Diving Activities may be conducted at a site that is remote, either by time or distance or both, from a recompression chamber and competent medical assistance. Despite this, I choose to proceed with these Diving Activities even in the absence of a recompression chamber and competent medical assistance.I understand that there are hazards and risks associated with travel to and from dive sites (“Dive Travel”), including, but not limited to the possible injury or loss of life as a result of a motor vehicle or watercraft accident.

    Regardless of the potential hazards and risks associated with Diving Activities and Dive Travel, I wish to proceed and I HEREBY PERSONALLY ASSUME ALL HAZARDS AND RISKS, including possible personal injury, loss of life and/or property damage, arising out of or in connection with Diving Activities and/or Dive Travel, whether foreseen or unforeseen, that may befall me while I am a participant in these activities, including, but not limited to, the academics, confined water, and/or Open Water Activities. I further release, exempt and hold harmless said program and Re- leased Parties from any claim or lawsuit by me, my family, estate, heirs or assigns, arising out of my enrollment and participation in this program including both claims arising during the program or after I receive my certification.

  • Standard Safe Diving Practices Statement of Understanding

  • I understand that as a diver I should:

     

    • Maintain good mental and physical fitness for diving. Avoid being under the influence of alcohol or dangerous drugs when diving. Keep proficient in diving skills, striving to increase them through continuing education and reviewing them in controlled conditions after a period of diving inactivity, and refer to my course materials to stay current and refresh myself on important information.
    • Be familiar with my dive sites. If not, obtain a formal diving orientation from a knowledgeable, local source. If diving conditions are worse than those in which I am experienced, postpone diving or select an alternate site with better conditions. Engage only in diving activities consistent with my training and experience. Do not engage in cave or technical diving unless specifically trained to do so.
    • Use complete, well-maintained, reliable equipment with which I am familiar; and inspect it for correct fit and function prior to each dive. Have a buoyancy control device, low-pressure buoyancy control inflation system, submersible pressure gauge and alternate air source and dive planning/monitoring device (dive computer, RDP/dive tables - whichever you are trained to use) when scuba diving. Deny use of my equipment to uncertified divers.
    • Listen carefully to dive briefings and directions and respect the advice of those supervising my diving activities. Recognize that additional training is recommended for participation in specialty diving activities, in other geographic areas and after periods of inactivity that exceed six months.
      Adhere to the buddy system throughout every dive. Plan dives – including communications, procedures for reuniting in case of separation and emergency procedures – with my buddy.
    • Be proficient in dive planning (dive computer or dive table use). Make all dives no decompression dives and allow a margin of safety. Have a means to monitor depth and time underwater. Limit maximum depth to my level of training and experience. Ascend at a rate of not more than 18 metres/60 feet per minute.
    • Be a SAFE diver – Slowly Ascend From Every dive. Make a safety stop as an added precaution, usually at 5 metres/15 feet for three minutes or longer.
    • Maintain proper buoyancy. Adjust weighting at the surface for neutral buoyancy with no air in my buoyancy control device. Maintain neutral buoyancy while underwater. Be buoyant for surface swimming and resting. Have weights clear for easy removal, and establish buoyancy when in distress while diving. Carry at least one surface signaling device (such as signal tube, whistle, mirror).
    • Breathe properly for diving. Never breath-hold or skip-breathe when breathing compressed air, and avoid excessive hyperventilation when breath-hold diving. Avoid overexertion while in and underwater and dive within my limitations. Use a boat, float or other surface support station, whenever feasible.
    • Know and obey local dive laws and regulations, including fish and game and dive flag laws.

    I understand the importance and purposes of these established practices. I recognize they are for my own safety and well-being, and that failure to adhere to them can place me in jeopardy when diving.

  • Sometimes the SuperDive Team will take photos or video recordings to memorialize participants' experiences. 

    Typical uses of these photos and videos include, but are not limited to: 

    • Sending digital or physical "Souvenir Prints" to program participants
    • Making photos and videos available in collaborative Google Albums
    • Creating educational or informational content
    • Creating promotional content

    I hereby authorize Superdive Scuba Inc., and those acting pursuant to its authority to:

    • Record my likeness and voice on video, audio, photographic, digital, electronic, online format or on any and all other media while participating in activities hosted by SuperDive Scuba Inc.
    • Use my name or social media handle in connection with these recordings.
    • Use, reproduce, publish, republish, exhibit, edit, modify, or distribute, in whole or in part, without requiring my inspection and approval, these recordings in all media without compensation for any purpose that Superdive Scuba Inc., and those acting pursuant to its authority, deem appropriate, including promotional or advertising efforts. These recordings may appear in a variety of formats and platforms now available and that may be available in the future. 

    I hereby release Superdive Scuba Inc. and those acting pursuant to its authority from liability, claims, and demands for any violation of any personal or proprietary right I may have in connection with such use, including any and all claims for liber, defamation, or invasion of privacy. I understand that all such recordings, in whatever medium, shall remain the property of SuperDive Scuba Inc. 

    I have read and fully understand the terms of this release.

  • As an added measure, we ask that this Agreement be signed by a third-party witness. This does not have to be an official notary but should be someone who:

    • Is over 18 years of age;
    • Is of sound mind;
    • Is not under the influence of drugs;
    • Is not a party to the document or have any financial interests in the document; 
    • Has known you for at least one year OR have taken reasonable steps to verify your identity.
    • Could be contacted at a later date to verify the authenticity of the signed agreement.
