If we have availability for the program, we will contact you using the information on this form. Filling this form out DOES NOT guarantee you a spot for the upcoming Fall Sale.

We will notify you by October 1 if space opens up for VIP. In the case that we can't take your items as a VIP customer, you will still have plenty of time to enter and tag your own items as a regular consignor. Please check out our website for tips and tricks to tagging: www.fourmamasmjsale.com
