To be used to apply for the CHTT/CMS/CPI/CSP/CST recertification.
Please make sure to read everything before submitting this form. Including double check that you are checking the correct certification, that you meet the requirements, and that you are up for recertification in 2024.

Please submit this form and attached proof of 24 hours of CEU credits for CHTT, CMS, CSP, and CST. CPI requires proof of 9 hours of CEU credits in order to be eligible for recertification.

In order to receive additional details on Exam testing and your ultimate pass/fail results, please allow PHTA emails to be approved communications to your individual email listed on this application.

In order to determine your continued eligibility for the designation of CHTT Certified Pool & Spa Hot Tub Technician®, CMS Certified Pool & Spa Maintenance Specialist®, CPI Certified Pool Inspector®, CSP Certified Service Professional®, or CST Certified Pool & Spa Service Technician®, please respond to the questions listed below. PHTA may require you to provide additional information to clarify your answers before PHTA can grant your recertification.
