Competitive U.S. Soccer Licenses Request Form

Please complete if your club is interested in hosting a Competitive US Soccer Coaching license course event. Once the application is complete our Cal South will follow up to set up course and instructors for your club event.
Please read and fill in the information in its entirety to ensure proper registration for your coaching event.

Requirements to host a 9v9 In Person or 11v11 In Person License
- This form must be submitted at least 30 days prior to your event date
- Minimum of 15 participants
- Classroom, field, lights (if applicable), bathrooms, first aid kit
- Host at event to provide permit if needed
- Host must provide one (1) team for the 2-hour field portion
- Total Duration: 2 hours field training | 2 hour lecture (In person or virtual)
Requirements to host a National D License
- This form must be submitted at least 30 days prior to your event date
- Minimum of 15 participants
- Classroom, field, lights (if applicable), bathrooms, first aid kit
- Host at event to provide permit if needed
- Host must provide one (1) team for the 2-hour field portion
- Total Duration:
10 virtual meetings and 4 days in-person meetings, (2 hr on the field each day, 5 hr in the classroom each day).
