Required Documentation

Paultons Park will accept the following as valid documentation for acceptance of the Queue Assist Scheme, your documentation must state the nature of the disability:
- A current and valid Access Card including the queuing logo on it. These cards are available here (www.accesscard.org.uk)
- Letter of diagnosis from a GP.
- Any other official document that states the disability.
Please upload at least one of these documents using the options below.

Please note that DLA letters, together with blue or orange badges, will not be accepted as these do not state the nature of the disability.

By ticking the above box I confirm that:

a.       I consent and agree to the following points set out in these Terms and Conditions.

b.       If the application is for someone else, I am either the legal guardian of said person or I have received their express permission to apply on their behalf.

1.       Paultons Park Ltd requires specific personal information from the applicant, this includes sensitive medical information and may include the following:

1.1.    a valid Access Card with the queueing logo specifically detailed

1.2.    a Personal Independence Payment (PIP) stating that you are entitled to a higher rate or enhanced rate mobility allowance for the applicant.

1.3.    a GP’s letter, Association Membership details, Council run membership or any other official document that states the nature of the disability

2.       Paultons Park Ltd requires this specific personal information listed above to ensure that the applicant qualifies for the Queue Assist Scheme.

3.       Paultons Park Ltd will not pass on or share any of this specific personal data with any person or company other than JotForm, a form and applicant management system. JotForm will store the specific personal data for Paultons Park to view and assess each application. The data will not be saved in any other location other than JotForm’s EU servers which are fully secured and encrypted, you can read JotForms GDPR compliancy information here. The data will be archived 30 days after the submission is received and then permanently deleted 30 days after that. No documentation will be downloaded and saved to another location.

4.       Only specific persons employed by Paultons Park will be authorised to view this specific personal data, these persons will be medical professionals and part of the Park’s own First Aid Nursing team.

5.       I understand that I am able to withdraw this consent at any time. However, in doing so will prevent Paultons Park Ltd from processing my application and if I change my mind will need to apply again using the online form.
