Your agreement to pay for veterinary services is implied upon treatment or admission.
• This Service Agreement is in effect for all veterinary services performed by Rood and Riddle Equine Hospital (RREH) on all patients owned/managed by me.
• Payment in full is required when the horse leaves the hospital unless other payment arrangements have been made with the RREH business office. The credit card below will be charged weekly for new clients with internal medicine cases, surgical colics, and septic joints.
• Payment options are: Cash, Check, Electronic Check, Major Credit Card (Visa, MasterCard, American Express or Discover Card) or CareCredit® Payment Plan.
• Any remaining account balance is due and payable at time of service. If current balance is not paid within 30 days of invoice, RREH is authorized to charge the credit card below for the entire balance.
• Overdue accounts are subject to a service charge of 1.0% per month or 12% per annum. Accounts older than 90 days will receive no further services until paid in full.
• If there is any dispute over work performed, ownership or billing, client has 10 days from invoice date to reconcile the dispute with RREH business office.
• I agree to pay any and all reasonable attorney’s costs and fees, if any, incurred in collection of clients’ account.
• I acknowledge and agree that any gathering of previously non-comingled horses, including farms, vans, racetracks and veterinary hospitals, carries a risk of exposure to infectious and contagious disease. Further, horses hospitalized for treatment of one disease are more vulnerable to exposure to other pathogens that they may contract or carry to other horses or locations. I acknowledge that RREH uses reasonable measures, such as segregation of horses into multiple barns based on their presenting signs and bio- security protocols based on those signs, designed to minimize lateral exposure of horses to other horses while being treated. I hereby release and discharge RREH and (it’s directors, officers, employees, agents, affiliates, contractors, representatives, successors, and assigns) from any claims, demands, actions, liabilities, losses, injuries, illnesses, damages, costs or expenses (including without limitation attorneys’ fees), known or unknown, fixed or contingent, arising out of or related to any infection or contagious disease the owners horse(s) may be exposed to or contract while visiting RREH.
• All horses located at RREH are closely monitored for any signs of infections or contagious disease. For that purpose, I authorize RREH to perform diagnostic testing on the owners’ horse(s) if RREH reasonably suspects the horse(s) may have, be carrying or has been exposed to infection, contagious disease or contagious micro-organisms. The costs of such testing and related procedures or services shall be included in RREH charges payable by owner or agent.
• RREH contributes to advancing equine health and welfare by utilizing the information in the medical records of our patients. We use anonymized data gathered during the routine course of diagnosis and treatment. I consent to the use of this type of information for research purposes to improve equine health.
