If you have an appointment reserved and do not meet these guidelines, we ask that you reschedule your appointment to avoid a cancellation fee. Please do not book an appointment if you are not by the following; I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Flicker + Finch, LLC has put preventative measures to reduce the spread of the Coronavirus/COVID-19 and all other infectious diseases/infections. I further acknowledge that Flicker + Finch, LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19 or any other contagious disease/infection. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any other disease/infection may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families. I voluntarily seek services provided by Erin Ramsey and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment. I attest that: * I am not experiencing any symptoms of illness such as cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * I have not traveled internationally within the last 14 days. * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.  * I have not been diagnosed with Coronavirus/Covid-19 and have not yet been cleared as noncontagious by state or local public health authorities. * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. For your comfort and safety, please notify your stylists if you have allergies, any physical issues or disabilities, or if you are pregnant. If you experience discomfort or any other symptoms during the treatment, please alert your service provider immediately.

    Cancellation Policy:I understand that things come up. I value your time and know you value mine too. If you need to cancel your appointment, please be sure to notify me no later than 24 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee. To ensure our cancellation policy is upheld, we require a credit card to book an appointment. Any appointment not canceled within 48 hours of the scheduled date and time falls under my cancellation policy protocol. You will be charged 50% to your card on file of what the scheduled services were valued. If you have any questions regarding this policy, please communicate them before committing to an appointment.No Show Policy:A "No Show" is considered canceling the day of your appointment or not showing up. In the event that you cancel the day of your appointment or do not show up, 100% of the service will be charged. Saved Payment Method:All guests' payment methods are safely and securely stored on file using my online booking system, Vagaro. Service Adjustment Policy:Your satisfaction is the highest priority. If you are not entirely content, please let me know during or within seven days after your service has been performed.  I am committed to making any needed corrections, as long as it falls within the services discussed and agreed upon during the consultation. I will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within seven days of your initial appointment.Product Return Policy:The next scheduled appointment may return a very lightly used product you have with me. Make sure to reach out to me with your concerns as soon as possible after using the product​.Salon Etiquette:Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. Our staff has the right to refuse service to anyone behaving improperly, intoxicated, or if their state of physical or mental health may influence the effects of the service.Confidentiality and Privacy:Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies.

    ​By reading and agreeing to this statement, I have read and understood all of Flicker + Finch, LLC Policies found in the Digital Welcome Packet and stated in this memo. I agree to have my payment information saved and stored securely. I also understand that My Stylist and I will have an in-depth conversation about all aspects of today's appointment, which I agree to and acknowledge, including price and the expected physical and aesthetic results. I hereby release and agree to hold Amy Jodie / Flicker + Finch, LLC harmless from and waive on behalf of myself, my heirs. Any personal representatives any causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Amy Jodie/Flicker + Finch, LLC. I understand that this release discharges Amy Jodie/ Flicker + Finch, LLC from any liability or claim that I, my heirs, or any personal representatives may have against Amy Jodie or Flicker + Finch, LLC with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Amy Jodie. This liability waiver and release extends to the salon together with all owners, partners, and employees.
