Community Partner Book Bag Claim Form

Requests for book bags will be granted on a first-come, first-served basis however we may adjust quantities based on available inventory. PLEASE NOTE: THIS FORM IS NOT INTENDED FOR USE BY PARENTS/GUARDIANS.
As a communty partner with The Heart2Heart Foundation, please use this form to request book bags with basic school supplies for the families you serve. Your bags will be reserved and held for pick up on Friday, August 5 or Saturday, August 6 at the Rock Hill Galleria. DEADLINE: submit your request by June 30, 2022.
