If you have an instrument you would like to donate to the LA Phil and it's learning programs, please fill out the following form so we can get a sense for the value and type of instrument.

If you require a tax receipt, you are responsible for obtaining a qualified appraisal of the instrument to be filed with your 8283 form. Should you have questions, please consult with your tax advisor.
